Objective:To evaluate the treatment effect of microsurgical resection of acoustic neuroma by subocipital retrosigmoid approach using neurophysiological monitoring.Methods:The clinical date of94cases of microsurgery excision of acoustic neuroma treated via suboccipital retrosigmoid approach were analyzed respectively. All patients underwent intraoperative neurophysiological monitoring.Results:93cases of patients were resected totally(98.9%);1cases were resected subtotally(1.1%); Facial never was preserved in all cases anatomically (100%); Petrosal vein was preserved in91cases anatomically(96.8%);Listening was retained in14cases/71cases(19.7%); No deaths was reported.Conclusion:1. Acoustic neuroma can be cured by microsurgery safely and effectively.2.Removal of acoustic neuroma via suboccipital retrosignoid-trans internal auditory canal approach,with meticulous dissection and in conjunction with intraoperativ neurophysiological monitoring is of great importance to obtain a higher rate of total resection,as well as facial and auditory nerve, petrosal vein preservation,precautious and immediate management of postoperative complictions is indispensable to obtain a satisfactory outcome.3.The size of tumor is a significant predictor of postoperative facial nerve function.4.To the experienced surgeon,microsurgical treatment is still the first choice for the small acoustic neuroma.
