Splint And Arthrocentesis Of Tmj For The Treatment Of Anterior Disc Displacement Without Reduction

Objective:Anterior disc displacement without reduction (ADNR) is one of the commonly encountered diseases of temporomandibular disorders. It causes the limitation of open mouth and pain in opening mouth. We suffered 48 patients from anterior disc displacement without reduction in Stomatology Hospital of Shandong University, and treated them with splint, arthrocentesis and splint united arthrocentesis. We analyzed and summarized the clinical curative effects of the three methods, and tried to explore reasonable therapeutic method of anterior disc displacement without reduction. Methods:48 patients were divided into 3 groups. It is: A group, splint treatment; B group, arthrocentesis; C group, splint united arthrocentesis. We measured the clinical indexes of prior and post treatment. The clinical indexes are pain index and maximal opening of mouth. Compare the curative effects of the three methods by statistical analyzing the clinical indexes. Results:(1)pain indexes: Compare the change of the three groups' pain indexes of prior and post treatment, we fund splint treatment group < arthrocentesis group < splint united arthrocentesis group. And they have significant difference after statistical analysis(P < 0.05).(2) maximal opening of mouth(MMO): Compare the change of the threegroups' MMO of prior and post treatment, we fund the change of splint treatment group was lower significantly than arthrocentesis group and splint united arthrocentesis group(P < 0.05), and the change of arthrocentesis group and splint united arthrocentesis group has not significant difference(P > 0.05). Conclusions:(1) About the treatment of anterior disc displacement without reduction, the three methods are feasible and efficacious.(2) The curative effect of splint united arthrocentesis is better than splint treatment and arthrocentesis, and meanwhile arthrocentesis is better than splint treatment in excellence of curative effect.
