Objective We evaluated the long segment pedicle screwinternal fixation (SPSIF) used in spinal injury in order to instruct thefuture management of these patients by reviewing 313 cases fromAugust 2000 to March 2005, among which 208 patients got operated.Methods The patients were divided into 4 groups by thefracture type: simple compression fracture group, burst fracture group,dislocation group and multiple segment spinal injury group. Wecontrasted the effect of short SPSIF with the long SPSIF in burstgroup and dislocation group.Results The X-ray plain after operation showed that long andshort SPSIF both could rearrange the sequence of vertebrae of patientswith simple compression or burst fracture, while CT told that thedecompression and the shape of the rearranged vertebrae were good,and there was no difference between the short and the long SPSIF.However, there were 5 patients applied with short SPSIF whosevertebrae shifted somehow, while the long SPSIF seemed steady. Andthe long SPSIF showed lower incidence in loose and rupture of thescrew than short SPSIF.Conclusion The long SPSIF is an effective way to spinal injurywith lateral shift, anatomic reduction to the lateral and sagittal shiftbefore fixation and bone graft fusion are necessary.
