Carpal tunnel syndrome (CTS) is a compression neuropathy of the median nerve caused by the increased pressure within the carpal tunnel. It is a pervasive peripheral nerve disease in the field of hand surgery and occurs more in females than in males. It is characterized by nocturnal hand discomfort, numbness and pain of fingers in the median nerve distribution, and relief after shaking or rubbing the hand. This study reviews the evocable factors in the anatomic and physiological aspects of CTS, the pathologic changes in CTS, the clinical manifestations, electrophysiological changes, clinical diagnosis,differential diagnosis and methods of operational treatment of CTS. The endoscopic treatment technique of CTS namely the endoscopic carpal tunnel release (ECTR) is the focus in this study. This article simultaneously introduces the endoscopic operation processes, indications, advantages, contraindications and complications of ECTR in detail. Form April 2003 to November 2004, 11 essential patients (13 cases) were treated by two-portal half endoscopic carpal tunnel release. The subjects were all females, ages varied form 43 to 68.
