Objective: To investigate the clinical application with diagnosis of Digital subtract angiography (DSA) in intracranial aneurysmal Subarachnoid hemorrhage( SAH).Methods: Retropective analysis was made on the clinical data of 86 cases highly suspected to the intracranial Aneurysm(AN) which displayed spontaneous SAH on CT. Of 86 cases. Male 39, Female 47, age from 29—77 years, average 49. all of 86 cases were examined by DSA. The cerebral angiographies were taken by Seldinger technique through right femoral artery. DSA of 86 cases were displayed distinctly with routine anterior-posterior projection and lateral projection alone, or in combination with oblique projection.Result: Out of 86 cases ,41 cases DSA were performed within 3 d ays, s light vascular spasm were shown in 7 cases. A pparent vascular spasm were shown in 9 cases when DSA performed after 3 days and within 2 weeks. 4 cases of 9 cases were found AN by repeated DSA after 2 weeks. 45 cases were performed DSA after 2 weeks, of 45 cases 11 cases performed for the second time, only 8 cases shown vascular spasm , apparently ease compared with DSA afer 3 days and with in 2 weeks.Among all 86 cases, 79 cases were diagnosed AN, 82 AN were found in all, 1 AN in 76 cases, 2 AN in 3 cases. 8 AN located at internal carotid artery, 3 AN located at ophthalmic artery, 34 AN located at posterior communicating artery, 1 AN located at anterior choroidal artery, 6 AN located at anterior cerebral artery, 15 AN located at anterior communicating artery, 6 AN located at middle cerebral artery, 2 AN located at basilar artery, 1 AN located at posterior inferior cerebrallar artery, 6 AN located at posterior cerebral artery. 7 cases were not found AN.Conclusion: DSA is the most effective method in diagnosis of intracranial aneurysm. If the DSA of spontaneous SAH were performed within 3 days or after 2 week, particular oblique projection were selected and the DSA were performed repeatedly, the diagnostic accuracy of intracranial aneurysm would be greatly improved. |
