Objective In recent years, Liver transplantation(LT) have been developed rapidly in China and have became one of the most important therapeutic option for patients diagnosed with hepatocellular carcinoma (HCC), but recurrence and metastasis of HCC is the most important risk factor of short-term postoperative death, which have been restricting LT for HCC to develop forward. It had been confirmed that the expression of P27, P53 and FAK play important roles in metastasis of HCC after liver resection. By analyzing HCC specimen from the patients who received LT in our transplantation center from June, 2002 to December, 2003 to explore the expression of protein P27, P53 and Focal Adhesion Kinase (FAK) in HCC and their roles in the recurrence and metastasis of HCC for effective directing the clinical work.Materials and methods 30 patients with HCC underwent transplantation in our hospital were divided into two groups according to the term of recurrence and metastasis after transplantation: recurrence and metastasis in early stage (<6months RMES) and in medium-late stage (>6months RMMLS), there are 12 of RMES and 18 of RMMLS. Immunohistochemistry (IHC) was performed to investigate the expression of P27, P53 and FAK of HCC samples from the patients, the relationship between the expression of P27, P53, FAK and the recurrence and metastasis of HCC were analysed. The results were dealed with Chi-square test and logistic regression analysis.Results The different degree of expression of P27, P53 and FAK was observed in all specimens. The expression of P27 are significantly lower in the group of RMES (P<0.05), and no difference was found for the expression of P53 between RMES and RMMLS(P>0.05), but Chi-square test for trend showed significant different(P<0.05); the analysis of combination of P27 and P53 showed the synergetic effect for RMMLS(P <0.05 for trend); the difference of the expression of FAK was not observed in the two groups(P>0.05).Conclusions The reduced expression of P27 may contribute to early recurrence and metastasis of HCC after transplantation. P27 maybe an important marker for evaluation of the prognosis of HCC patients underwent LT. The analysis of combination of P27 and P53 could behelpful to evaluation the recurrence and metastasis of HCC after transplantation. The meanings of FAK's expression in the recurrence and metastasis of HCC after LT hasn't been found, which need to be researched more.
