| Transcranial Doppler is a practical techniques in the field of neurovascular. From 1982, It was used in clinic diagnosis and research of science,because of it's characteristic of noninvasive,easy to operate and repeated. This study research in timing,location and how to gain a beter understanding of intracranial arterial hemodynamics by using TCD in clinic.This study include 33 consecutive patients of intracranial aneurysms who had suffered a recent SAH in our department from January 2003 to December 2003. The TCD examination should be performed before neurosurgery operation and 3-7,10,14 days in the postoperative course in all patients. Special attention was paid to the blood flow velocities of MCA and eICA. In the following study, we analysed the clinical outcome, neuroradiological features, and TCD findings in 33 patients after rupture and operative treatment of intracranial aneurysms.In 33 patients, there are 15 patients have symptomatic vasospasm after operation. And we found in these patients, the TCD velocity of MCA is large day-to-day increases during Days 3-7 after operation. The precranial segment, just proximal to where the artery enters the carotid canal, can be assumed to have constant diameter and is clearly not influenced by cerebral vasospasm. Therefore, in this segment changes in velocity will be linearly related to changes in flow. The observed reduction in velocity could, therefore, be attributed to a proportional reduction in hemispheric cerebral blood flow.In conclusion, TCD can diagnosis cerebral arterial vasospasm in the postoperative course of intracranial aneurysms safty,quickly and noninvasive. But TCD can't give exact calculation of delayed neurological deficits after cerebral vasospasm. |
