A Study Of Laminin And Laminin-receptor In Cervical Intraepithelial Neoplasia And Carcinoma Of Cervix

Cervical cancer is a common malignancy which threatens women's lives.But we can't diagnose it early enough and know too little about the machanism of it's occurrence and development. This study is about to investigate the relationship between it and laminin and laminin-receptor. Maybe this study will be help to know more about the the machanism of it and help to diagnose it earlier. Object To investigate : 1 The relationship between laminin in serum and cervical intraepithelial neoplasia (CIN) and cervical carcinoma. 2 The relationship between cervical intraepithelial neoplasia and cervical carcinoma and the distribution of laminin-receptor(LN-R) and basement membrane (BM) laminin﹑epithelial cell laminin in cervix. 3 The relationship between laminin in serum and the distribution of laminin-receptor(LN-R) and basement membrane (BM) laminin﹑epithelial cell laminin in cervix. Methods Venous blood was drawn from 10 women with normal cervix (control group) and 62 cases of women with CIN or cervical carcinoma(study group).We collected venous blood before and 1 week after the operation or radiotherapy, and determined consistency of laminin in serum by enzyme linked immunosorbent assay(ELISA). At the same time the distribution of laminin-receptor(LN-R) and basement membrane (BM) laminin﹑epithelial cell laminin in cervix was investigated in normal cervix and abnormal cervix by immunohistochemistry . Results 1 The relationship between laminin in serum and cervical intraepithelial neoplasia and cervical carcinoma: (1) Before the treatment , the consistency of laminin in serum of patients with CINIII or invasive carcinoma was higher than that of woman with normal cervix (P<0.05)； The serum laminin consistency of the patients with malignant cervical tumor was clearly related to the degree of histological differentiation of the tumor. (2) The serum laminin consistency of the patients in the study group went down to normal after the therapy(P<0.05)to normal (p>.10). 2 Epithelial structures in normal cervix and CINI-II were surrounded by a generally intact basement membrane,i.e.BMI. There wasn't any LN in epithelial cells; Epithelial structures in CINI-III were surrounded by a generally intact basement membrane,i.e.BMI, while LN was founded in epithelial cells ;The expression of laminin in cervix was related to the degree of histological differentiation of the tumor(χ2=6.54,P<0.05).3 A significant difference of the expression of LN-R existed between the women with normal cervix and the patients with invasive cervical carcinoma. LN-R wasn't found in normal cervix ,while it was found in invasive carcinoma and not only related to the degree of tumor cell differentiation(χ2= 384, P2<0.005) but also to the lymph node metastasis(χ2=10.19, P3 <0.005). 4 The relationship between the expression of LN-R﹑LN in cervix and the serum laminin consistency: the serum laminin consistency was related to the expression of both LN-R and LN in cervix .5 The level of LN-R in cervix was higher when the level of LN in intraepithelial cells increased and there was a significant difference of the level between BMI and BMII-III (χ2=7.9, P<0.05).Conclusion 1 This study suggested the serum laminin consistency was normal in women with normal cervix or cervix withnot malignant tumor. The consistency increased in women with malignant cervical tumor especially with poorly differentiated tumor .So maybe we could diagnose malignant cervical tumor by determine the serum laminin consistency of the patients who properly with malignant cervical tumor .2 The immunostaining pattern of LN and LN-R in cervix suggested in malignant cervical tumor the synthesis of laminin in tumor cells increased while the absorption of laminin in basement increased especially in poorly differentiated tumor .3 The serum laminin consistency was related to the expression of laminin both in intraepithelial cells and in basement.
