Clinical Study On Endoscopic Thyroidectomy By The Via-axillary Or Via-Breast Approach

Objective:To analyze the advantages and shortcomings of two methods of laparoscopic thyroidectomy by comparing the levels of hydrocortisone, blood sugar, GRP in the two kinds of operations, and comparing operative time length, the time length for creating the subcutaneous space, the post-operative degree of pain, the volume of drainage fluid and patients' degree of satisfaction to the wounds in the 3rd and 6th month after the operations. This study is intended to offer some evidence for surgeons to choose the method for laparoscopic thyroidectomy.Method:The patients were all chosen from our hospital according to the intaking and rejecting standard. The patients were divided into two groups. Either group contained 20 patients. The WBC count and the levels of CRP were tested at 8:00am on the day before the operations and at 8:00am on the 1st, 2nd and 3rd day after the operations. The levels of hydrocort isone were tested at the following time: 8:00am on the day before the operations, 60 minutes after the operations began, 8:00am on the 1" and the 3rd day after the operations. The fasting blood sugar was tested at 8:00am on the day before the operations, 60 minutes after the operations began, 2 hours after the operations and at 8:00am on the 1st day after the operations. The overalI drainage fluid volume was recorded. The degree of wound pain was recorded at 8:00am on the 1st, 2nd and 3rd day after the operations. The degree of patients' satisfaction on the wounds was also recorded in the 3rd and 6th month after the operations. All the data were analyzed with SPSS 10.0 software:(1) The data of hydrocortisone, blood sugar, CRP, WBG count and degree of pain in the wounds were analyzed with variance analysis respectively, in order to compare the intro-group and inter-group difference.(2) The operative time length, the time length of creating the operative space and the overall volume of drainage were analyzed with independent sample T test respectively, in order to compare the inter-group difference.(3) The degrees of patients' satisfaction on wounds in the 3rd and 6th month after the operations were analyzed with rank sum test.(4) Based on the data of hydrocort isone, CRP, WBC count, and the degrees of pain, we draw a box - curve, in order to compare the distribution of data more di rect ly.Result:(1) The levels of hydrocort isone, WBC count and CRP had significant difference at difference time in either group (P<0. 05) , but there was no significant difference between the two groups (P>0. 05) .(2) The levels of blood sugar had significant difference at difference time in either group (P<0. 05) , and there was significant difference between the two groups (P<0. 05) .(3) The degree of pain had significant difference at difference time in either group (P<0. 05) , and there was significant difference between the two groups (P<0. 05) .(4) There was no significant difference between the operative time length of two groups (P>0. 05) . The time length of creating the operative space, the overal I drainage volume and the degree of patients' satisfaction on the wounds i n the 3rd and 6th month after the operat i ons had s i gn i f i cant d i f f erence (P<0. 05) .(5) The levels of hydrocort i sone in two groups rise the most remarkably 60 minutes after the operations began, and went down to normal on the 3rd day after the operations. The level of CRP rise the most remarkably on the 2nd day after the operations and descended from the 3rd day after the operations. The WBC count rise the most remarkably on the 1st day after the operations, and descended from the 2nd day after the operations. The level of blood sugar rise significantly 2 hours after the operations began, and descended from the 1st day after the operations. In the via-breast thyroidectomy group, there was more significant pain, slower recovery, more drainage and lower degree of satisfaction on the wounds after the operations.Conclusion:From the comparisons of the data of the via-breast laparoscopic thyroidectomy group and the via-a...
