| Objective To investigate the relative factors of periodontitis in patients with non-insulin-dependent diabetes mellitus(NIDDM) and provide the preventing and controlling basis for them. Methods The 370 patients with NIDDM were devided into 3 groups according to the American Academy of Periodontology(AAP) standard of chronic periodontitis: advanced periodontitis (140 cases) ,slight to moderate periodontitis( 94 cases) and central group (136 cases). They all were investigated face-to-face , their sex ,age, DM family history, chronic disease family history ,smoking ,diabetic duration ,health instruction , oral hygiene instruction, plasma glucose under fasting (FBG), HbAlc ,total cholesterol( TC) , triglyceride (TG) , high-density lipoprotein cholesterol( HDL-c) , low-density lipoprotein cholesterol( LDL-c) , ApoAl , ApoB and lipoprotein-a were analysed. A pair-matched case-control study was carried out. Data was analysed with conditional Logistic regression.Results In multiple Logistic regression : smoking , macrovascular complications peripheral nervous complications , dental calculus , HbAlc , TC and TG showed significantly independent association with periodontitis, the protect factors were diabetes health instruction and oral hygiene instruction . The odds ratio OR and OR 95%CI were 5.397C2.181-13.354), 9.008 (1.856-43.705), 41.294 (1.503-1134.342), 3.192(1.298-7.848), 7.082(2.995-17.946),6.511(2.202-19.253), 3.628(1.334-9.872), 0.137(0.025-0.771). Compared with periodontitis , advanced periodontitis also had relation to retina complications(OR=7.943, 1.727-36.537) and diabetes health instruction (OR-0.052, 0.003-0.072). Conclusion We should strengthen health instruction and oral hygiene instruction and give up smoking instruction , control abnormity metabolizability to control and prevent periodontitis in patients with non-insulin-dependent diabetes mellitus(NIDDM). |
