CLINICAL STUDY ON THE LELATIONSHIP BETWEEN TCM SYNDROMES OF DYPE 2 DIABETES MELLITUS ANDINSULIN RESISTANCEMajor: Clinical of internal medicine ofChinese traditional Postgraduate: Lai Chang Qin Tutor: Prof. Fan De Rong Prof. Zhou Gu YingABSTRACTBackground: TCM syndromes differentiation is the marrow of TCM. 1R is the main clue and basic characteristic of DM.Objectives: To research on the changes of INS and TNF- a , HbAlc BMI and WHR.Materials and methods: 57 patients of type 2 DM were selected as research subjects, among which 28 is male and 27 is female. The average age was 59.58 ?12.59 years old. According to the differentiation standards of DM of TCM, 25 patients belonged to deficiency of Yin and excess of heat syndrome; 27 patients belonged to deficiency syndrome of Qi and Yin; 5 patients belonged to deficiency syndrome of Yin and Yang; 26 patients belonged to heat-blood stagnation syndrome; 11 patients belonged to phlegm and turbid stagnation syndrome; 24 patients belonged to syndrome without heat-blood and phlegm and turbid stagnation. As the same time 26 healthy people were selected as controls, among which 14 is male and 12 is female. The average age is 58.8 ?12.5 years old. FPG was measured using means of zymology. FINS and TNF -a were measured using RIA. HbAlc was measured using analytical means of tiny post ply. And measure weight, stature, waistline and buttock line. Then count out IAI [IAI=1/(FPG x FINS)] > BMI {BMI=[weight(kg)/stature(m2)]}and WHR [ WHR=waistline(cm)/buttock line(cm)].Results: (1) the IAI of 57 patients of DM was significantly higher than that of the normal controls (P<0.05). The IAI of the healthy controls wasthe highest, then the deficiency syndrome of Yin and Yang, the deficiency syndrome of Qi and Yin, the deficiency of Yin and excess of heat syndrome followed in sequence. The IAI of the deficiency syndrome of Yin and Yang was significantly higher than mat of the deficiency syndrome of Qi and Yin and the deficiency of Yin and excess of heat syndrome (PO.01). The level of FINS of the deficiency of Yin and excess of heat syndrome was higher than that of another. The level of FPG of the deficiency syndrome of Qi and Yin was higher than that of another. (2) The process of the disease of the deficiency syndrome of Yin and Yang was significantly longer than the deficiency of Yin and excess of heat syndrome (P<0.01). The process of the diseases of the heart-blood and phlegm and turbid stagnation syndrome was significantly longer than the syndrome without heart-blood and phlegm and turbid stagnation (PO.01). (3) The levels of TNF- a. HbAlc, WHR, BMI of 57 patients of DM were significantly higher than those of the healthy controls (P<0.01). Among which the levels of TNF-a , HbAlc> BMI of the heart-blood and phlegm and turbid stagnation syndrome were higher than those of another. (4) According to statistical analysis, the levels of TNF-a v HbAlc> WHR were positively correlated to IAI of DM and the process of the disease was negatively correlated to IAI of DM. The level of TNF- a was negatively correlated to the process of the disease * WHR and BMI.Conclusion: (1) IR is closely associated with TCM syndrome differentiation of DM. (2) IAI may serve as a reference index of TCM syndrome differentiation of DM. (3) TNF- a , HbAlc, WRR, BMI may serve as reference index of the heart-blood and phlegm and turbid stagnation syndrome of DM. (4) TNF- a , HbAi c must be closely related to the development of IR of DM.
