| Background and Objective:Rheumatoid arthritis(RA) is a kind of multi-systematically inflammatory autoimmune disease, mainly relating to ambient joints. The reason of the disease is uncertain and its pathologic change is chronic inflammation of joints, cell infiltration, information of pannus and the erosion of cartilage and bones, which lead to the destruction and dysfunction of joints structure. Because of its high incidence of disease and rate of disability, it has been one of the international tackled problems. The treatment of RA aims at relief of pain, prevention of disability, maintenance of joints function and improvement of the standard of patients living. WHO has divided the drugs to treat RA into improving symptoms and controlling the development of disease. It is a golden rule for clinic treatment. However, due to its uncertain pathogeny, there is no specific and pertinent drug. In Chinese medicine, RA belongs to the range of "arthalgia syndrome"; it is mainly caused by invasion of wind,cold,dampness on the condition of deficiency of qi and blood or kidney and liver; its pathology is the syndrome of phlegm accumulating with stagnant blood. Mainly on the basic of planning treatment according to diagnosis and through multi-layer, multi-loop, multi-method and multi-target way, it has great advantage to anti-inflammatory,analgesia, regulating immune function and prevent the development of disease. The research topic lies on the experience of national famous Professor Wang Lvqiu and mentor Professor Wang Yue. We develop the "BiTongLing" infusion, which is on the basic of their theory and experimental prescription. We inquire into its curative effect and principle both in clinic and experiment.Methods: Clinical Research: Sixty RA cases, which fit the ARA diagnosis standard (1987), are divided evenly into two groups at random.The patients of treatment group take orally "Bitongling" infusion 1 sachet td and the patients of control group take orally "Fengshigutong" capsule 4 capsules bd. During eight-week treatment, we observe the curative effect and the main symptoms and signs, such as arthralgia, arthrophyma,morning stiffness,the power of grip and VAS test.We also test ESR,RF,CRP and T cell subgroup of patients. Experimental Research: There are four parts as follows.â‘ We estimate the MTD of mice by observing the response of miceto "Bitongling" infusion.â‘¡We observe the "Bitongling" infusion's effect on the model of adjuvant arthritis of rats from the degree of rats' swollen feet , the weight of model rats and model rats' thymuses and spleens.â‘¢We induce T and B lymphocyte cell multiplication reaction of adjuvant arthritis of rats by ConA and LPS respectively.â‘£Observe the analgesia effect by two ways. Results: The clinical total effective rate is 93.3%, which significantly surpasses the effective rate (76.6%) of control group. After treatment, the main symptoms and signs are much better than before. The sensitive parameters, such as ESR,RF and CRP have been lowered to some extent or got back to the standard level. "Bitongling" infusion has double function of inhibiting humoral immunity and regulating cell mediated immunity; it can greatly lower the level of immunoglobulin (IgA, IgG, IgM), regulate T cell subgroup and keep the balance of Th/Ts. The experimental research shows four results as follows.â‘ The MTD of mice been taken orally "Bitongling" infusion is 196.8g/kg,which is 157 times the daily dosage of adults.It shows "Bitongling" infusion is safe.â‘¡It can greatly lower the rats' swollen feet, inhibit the decrease of rats'weight and relief the weight of model rats' spleens and thymuses.â‘¢"Bitongling" infusion inhibits T and B lymphocyte cell multiplication reaction.â‘£In addition, it can also prolong the pain threshold of mice and inhibit body's turning reaction times.
