IntroductionProiiferative vitreoretinopathy (PVR) is the main cause of failure of rhegmatogenous retinal detachment (RRD) operation. It may cause severe comph"cations resulting in blindness. Previous studies demonstrated that the proiiferative membranes consist of different types of cells such as retinal pigment epithelial cells, fibroblasts, glial cells, and macrophages , and different types of extracellular matrixes. On the other hand, some other studies demonstrated the presence of immune cells, immunoglobulins, and complement proteins in the epiretinal membranes implicating the involvement of the immune system. The participating cells influence tissue proliferation. In multicellular tissues such as epiretinal membranes, cell - cell interactions may be a major factor in the control of cell migration, cell recognition and adhesion , cell differentiation and cell proliferation. Intercellular adhension molecule - 1 ( ICAM - 1) is one of the immunoglobulins superfamity which is considered to be important for leukocyte migration through endothelial cell and into the surface of the vessels.We have made advances in understanding of the molecular mechanisms involved in the pathogenesis of PVR. The adhension action of cell - cell or cell - extracellullar matrix is a basic and important prob-lem to the PVR. This study is to measure subretinal fluid levels of intercellular adhension molecule - 1 (1C AM - 1) in eyes with rhegmat-ogenous retinal detachment complicated by PVR to investigate whether the levels of ICAM - 1 are involved in the pathogenesis of PVR and provide theory base of treating PVR in the futrue.Materials and Methods1. patientsThere are 39 patients with RRD complicated by PVR, including 21 males and 18 females. They are divided into grade A,B,C,D according to PVR grades criterion. (The patients in gradeA were too few to be listed in this research)2. samples collectionThe SRF was aspirated with a 1ml syringe from the eyes during drainage surgery.Eight cadaveric vitreous samples were taken from volunteer donor eyeballs for keratoplasty as normal control specimensAll the samples were stored at -70 until use3. assay of ICAM - 1Sandwich Enzyme - linked immunosorbent assay kits ( shenxiong limited co. shanghai) was used to detect the ICAM - 1 in the samples. The standard reagent were diluted in proper order, set up normal vitreous as control, the absorbance of the samples read at 490nm, levels of ICAM - 1 in the samples were interpolated from standard curves.4. statistical analysis Analysis of variance and t'test.ResultsSRF from PVR groups contained significantly higher levels ofICAM - 1 than vitreous from controls ( P <0.05) . The levels of IC AM-1 in eyes with RRD of < 4 weeks'were lower than those of > 4weeks'(t' < 0. 005). And the graver of PVR, the higher levers ofICAM - 1 in SRF.DiscussionThe pathogenesis of PVR is associated with a posttraumatic inflammatory responses. It goes through inflammatory course, proliferation period and scar period. Various cellular processes such as migration , proliferation and activation require cell - cell or cell - extracell-ullar matrix contact. These may be modulated by cellular adhension molecules (CAMs). ICAM - 1 is a important member of them. The raise of ICAM - 1 may promote the differentiation, migration of the RPE cell, fibroblast cell and lead to fibrosis and proliferation. Our studies shows that SRF from eyes with PVR contained higher ICAM -1 than controls, and the graver of PVR, the higher levers of ICAM -1 in SRF. This indicated that the levels of ICAM - 1 depends on the extent of action which is midiated by the cytokines at blood - retinal barrier.This study shows that SRF from eyes with RRD complicated by PVR contained higher levels of ICAM - 1 than those from normal control specimen. And SRF levels of ICAM -1 were related to the severity of PVR and the duration of RD. In our view, enhanced release of this molecule is dependent on cytokines - mediated cellular activation at the blood...
