The prognosis of gallbladder is very poor and the postoperation survival rate is low. Some studies indicated that surgery of gallbladder Cancer was associated with an overall 5 - year survival rate of 6.6%. Curative resection was possible in 35.5% of Cases. NonCurative Surgery revealed Poor prognosis with median Survival time Limited to 3.2 months. However,the mechanism underlying the development and progression of GBC is not fully understand. Caspase - 3 is a member of ICE family. It plays an important role in the programmed Ceadeath.BcL - 2 is the Cancer gene of follicular B - cell lymphoma. The [ BcL - 2 ] can stop releasipn of cytochrome C and restrict the activity of cpp32. Then restain programmed Celldeath. In Some studies,the expression of BcL - 2 and Caspase - 3 is abnormal at the gastric carcinoma, bladder Carcinoma and so on. There is relationship between their expression. But there is no resemble study in GBC. In this study, im-munohito - chemistry was used to detect the expression of BcL - 2 and Caspase - 3 in gallbladder Carcinoma, gallbladder adenoma and chronic cholecystitis, we analysis the relationship between the expression level and clinical data for the purpose to understand the role of BcL -2 and Caspase - 3 in the occurrence and development of gallbladder Carcinoma.Materials and MethodsTo collect 41 specimens of GBC, 13specimens of gallbladder adenomas, 9 specimens of chronic cholecystitis placed on the file. For the GBC,there were 23women and 18men,with a mean age of 58(range, 34to73 ). There were 15 better differentiated adenocarcinomas, 14moderately differentiated adenocarcinomas, 12poor differentiated adenocarcinomas (include undifferentiated adenocarcinomas ) , One squa-mous Cell Carcinomas ( better differentiatet). One adenosquamous Cell carcinoma (better differentiated) ,one gallbladder Sarcom(poor differentiated) ,One small cell Carcinoma (poor differentiated).Froe - micrometer - thick sections were cur from formalinfixed paraffin - embedded tissue block and mounted on polylysin - coated glass slides. The positive controls were section of Lymph node ( BcL -2) and tonsillar ( caspase - 3 ) tissue. Negative controls were sections stained without the primary antibody. Then plasma that have immuno-reactivity is yellow, buff or brown. For each sections, 10high - power (x400) micrsscopic fields were examined for 1000 Cell. BcL - 2/ caspase -3( + ) :greater than 10% of cells showed immunoreactivity; BcL -2/Caspase -3( - ) -.less than 10% of cells showed immunoreactivity. The data was processed with the SPSS software applying Chi -square test.ResultsFor the 9 chronic cholecystitis, 9 showed normal Caspase - 3 expression^ showed normal BcL -2 expression. All of 13 adenomasshowed normal Caspase - 3 ( BcL - 2 ) expression. For the 41 GBC, 19 showed normal Caspase -3 expression(46. 3% ) ,the rate of positive expression was significantly lower than benign lesion (P <0. 01). For the 41 GBC,14showed normal BcL-2 expression(34. l%)the positive rate was significantly lower than gallbladder adenoma and higher than chronic cholecystitis.For the 41 GBC, 19showed normal Caspase -3 expression (46. 3% ). The positive rate was significantly higher in better - differentiated Carcinomas than that in lower - differentiated cases. ( P < 0. 05). The rate of normal expression was closely associated with Nevin stage (P < 0. 05 ) and lymph node metastasis( P < 0. 05 ) , but was not associated with sex,age.There was significant association between the level of BcL - 2 expression and differentiation, Nevin stage and lymph node metastasis.For the 14 GBC that expressed normal BcL -21evel,13 showed normal Caspase - 3 expression (92. 9% ) ; For the 27 GBC that expressed decreased BcL-21evel,22 showed decreased Caspase -3 expression (81.5% ) . The correlation was found between the Caspase -3 and the BcL -2.DiscussionOver expression of BcL - 2 protein can restrict programmed cell-death, then improve occurrence and development of neoplasm.
