Coronary heart disease (CHD) is one of multiple etiology disease, which caused by both inherit and environment factors. Numerous epidemiologic studies indicated that major risk factors of CHD were hypertension, cigarette smoking, dislipidemias, obesity, diabetes mellitus, type A personality, family history of cardiovascular disease and abnormalities of coagulation and fibrinolysis system. The relationship between those factors are not entirely understood. Predispotion to CHD are variate in different population. In addition, take into consideration entire to date evidence of risk factors, the genesis mechanisms of CHD still can not be well explained. Obviously, further researches should focus on both defining new factors and understanding complicate relations between defined factors.Recent studies have suggested that periodontal disease is an independent risk factor for CHD. However, it is fare to state that in terms of pathophsiological interrelations, the global picture is far from complete. Therefore, we investigated whether the periodontal disease is associated with increased risk of CHD by a classical case-control study. We also examine therelationship between the changes of serum cholesterol in patients with CHD and periodontal disease.In case-control study, 146 CHD inpatients during July 2002 to October 2002 were enlisted in case group. The control group included 136 non-CHD inpatients and 131 healthy non-CHD population. The ratio of male and female was 133:13 in case group, which in control was 239:28. There was no significantly different between two groups in age, sex, education and profession.The prevalence of periodontal disease was 52.05% in CHD group, while 26.21% in control group. There were statistical significance in both group. Uni-analysis indicated that periodontal disease(OR=3.06(2.00 ~ 4.67)), cigarette smoking(OR=2.49(1.60~3.90)),hypertension(OR=2.40(1.59~3.79)), fat enriched diet(OR=l.93(1.28?2.90)), obesity(OR=2.01(1.22-3.30)) and family history of cardiovascular disease (OR=2.40(1.40?.12) may be the risk factors of CHD. Significantly dosage-effective response was also found in the relationship between periodontal disease and CHD( x 2=38.80, P<0.01). The differences between case and control group were not statistically significant for oral hygiene and the date when seeing a dentist lately. But people in CHD group considered they had poorer dental health than that of the same age. Multiple factorial Logistic regression analysis showed that periodontal disease, cigarette smoking, hypertension and family history of cardiovascular disease were the variables in the equation.The further dental examinations study on relationship between periodontal disease and CHD was conducted on 52 inpatients with CHD during July 2002 to March 2003 ,and a control group of 103 non-CHD individuals including 43non-CHD inpatients and 60 health non-CHD population. There was no significantly different between two groups in age, sex, education and profession. The prevalence of periodontal disease was 77.36% in case, which was 48.54% in controls. After a statistical analysis, there was significantly different between two groups (P<0.01). The presence of CHD was significantly associated with periodontal disease(OR=3.95(1.83~~8.53)).
