Objective:To study the changes of the r-CBF after trauma by comparing the SPECT cerebral perfusion image and CT image between post-trauma patients and normal people, and thus to provide clinical proof to the diagnose, treatment and prognostic analysis of " post-trauma cerebral ischemia and cerebral infarction". Materials and Methods:The SPECT cerebral blood stream tomography was performed to 30 cases of cerebral trauma and 12 normal persons, and the CT scan was also performed to cerebral trauma patients right after SPECT. The differences between the images of CT and SPECT were evaluated by eyes. The semi-quantitative method was operated to calculate the r-CBF through the SPECT images, and the results were analyzed statistically:(1)The positive rates of both CT and SPECT method to cerebral trauma were compared with chi-square method;(2) The r-CBF of focal lesions and relative lesions in normal peoplewere compared with t-test/ t'-test;(3) The r-CBF of focal lesions and of the opposite normal brain tissue were compared with paired t-test method;(4) The r-CBF of focal lesions in cerebral trauma patients of different grades were compared with one-way ANOVA method.(5) The relativity analysis of the r-CBF and GCS score of the focal lesions were performed.RESULTS:(l)The average positive rates of SPECT to cerebral trauma patients of the slight and middle grades are higher than those of CT.(2) Significant difference between the r-CBF of the cerebral trauma patients and the normal people were observed.(3) Significant difference between the r-CBF of the focal lesions and the normal brain tissue on the opposite side were observed.(4) Significant difference between the r-CBF of the focal lesions among different graded patients were observed.(5) The r-CBF and GCS scores of the focal lesions were found to be positively relative.CONCLUSIONS:(1)The cerebral ischemia is one of the basic pathological changes after cerebral trauma. The more serious the trauma is, the more deeply coma the patient is in, the more significant the cerebral ischemia is.(2) The SPECT can provide objective proof to the diagnoses ofconcussion of brain, post-cerebral-trauma syndrom and traumatic cerebral infarction.(3) The SPECT scan of r-CBF can be taken as an additional diagnostic criteria for typing of cerebral trauma patients.(4) In cerebral trauma cases, the sensitivity of SPECT is better than that of CT, while the specificity of CT is better than that of SPECT.
