The study on expression of heat shock protein 70 during the early period of orthotopic homologous kidney transplantationOBJECTIVE:To evaluate the fundamental character on expression of heat shock protein 70 during the early period of orthotopic homologous kidney transplantation,and corelation between heat shock protein 70 and rejection ofallograft.METHODS:(l).Model:Twenty four healthy adult rabbits were randomly pair-matched divided into two groups equally.Group I as autotransplantation of kidney group (control group). We first removed the rabbit's left kidney,perfuse with 4C H-CA perfusion solution and cold preservation,ready to practice orthotopic homologous kidney transplantation. We then removed right kidney and transplanted it to left kidney pedicle without perfusion;Group II as orthotopic kidney transplantation group.We first removed the rabbit' s left kidney,then transplanted the perfused left kidney of Group I to left kidney pedicle of Group II .At last we removed right kidney.Postoperation we transfused routinely and did not treated with immunosuppressive agent.(2).Detected blood uria nitrogen and blood creatinine of two group at before transplantation,24 hours after transplantation,72 hours after transplantation.(3).Detected the expression of heat shock protein 70 at before transplantation,1 hour after ischemia reperfusion,24 hours after transplantation,72 hours after transplantation,1 week after transplantation. RESULTS:(1). Blood uria nitrogen and blood creatinine:Group I has not significant change at before and after transplantation,Group II increased significantly at 72 hours after transplantation (P<0.001),has not significant change at before transplantation and 24 hours after transplantation.(2).Pathological section with hematoxylin and eosin staining:The fundamental character of rejection after rabbit allotransplantation was infiltration of monocyte and tubulitis,and severe endarteritis was unusual.(3). Immunohistochemical:The expression of heat shock protein 70 in normal kidney tissue was mild;At 1 hour after ischemia reperfusion,the expression of allograft's heat shock protein 70 in two groups were all increased significantly(P<0.001),and its localization existed mainly in nuclei and cytoplasm;There were not significant changes at 24 hours after transplantation on allograft's heat shock protein 70 in two groups;At 72 hours after transplantation,the expression of allograft's heat shock protein 70 in group II was increased significantly (P<0.001)-,There were not significant changes at 1 week after transplantation on allograft's heat shock protein 70 in two groups. (4).Western blot:The results was as same as immunohistochemical's results. CONCLUSION:(1). The expression of heat shock protein 70 in normal kidney tissue was mild;(2). The expression of allograft's heat shock protein 70 undergoing non-fatal injury(ischemia reperfusion et al.) was increased significantly . Its localization existed mainly in nuclei and cytoplasm;(3). At 1 hour after ischemia reperfusion and 72 hours after transplantation,the expression of allograft's heat shock protein 70 in group II were all increased significantly;(4).it is confirmative that there was correlation between the expression of heat shock protein 70 in kidney allograft and the rejection during early period of orthotopic homologous kidney transplantation. These studies also raise the possiblity of a functional role for heat shock protein 70 in th allogeneic immune response.And heat shock protein 70 maybe represent a prognostic marker for early graft rejection.
