OBJECTIVES: To investigate the common reasons and effective therapeutic methods for non-union. METHODS: Retrospective study by statistically analysing and evaluating the causes, therapeutic methods, therapeutic and functional results of 39 hospitalised patients who were suffering from non-union. RESULTS: Non-union contributed by inappropriate immobilisation presented in 18 cases (46.1%), whereas 19 cases (48.7%) were caused by infection. Of whom were treated by intra-medullary bone grafting combined with extra-medullary bone grafting, the bone union results were excellent in 27patients (75%), good in seven (19.4%), while the functional results were excellent in 21 patients (58.3%), and good in eight (22.2%). Average healing time was 4.6 months. The excellent and good rate of bone union of the infected non-union patients who were treated by first-stage autogenous fibular grafting was 86.7 percent, and the same rate was 60% in functional results. Average healing time was 4.88 months. CONCLUSIONS: Inappropriate immobilisation and infection are the most common reasons for bone non-union. Intra-medullary bone grafting combined with extra-medullary bone grafting is a reliable option in the management of difficult non-union. First-stage autogenous fibular grafting is safe, effective and practicably in treating septic non-union.
