Background H.pylori infection is the main cause of many gastric disease, and more and more studies show that it is greatly associated with gastric carcinoma. Apoptosis phenomenon is hotly researched recently, and the relationship of H.pylori infection to gastric epithelial cell apoptosis and proliferation has been highly regarded now.Objective To investigate the effect of H.pylori and different bacterium strains (cagA+ and cagA-) infection to the epithelial cell apoptosis and proliferation, and observe the expression change of the Fas/FasL, Bax and Bcl-2.Methods Gastric biopsy specimens were taken from 80 patients, these patients were divided into 9 groups according to the H-E stains and H.pylori infection. And H.pylori(+) were subdivided into cagA+ and cagA-. RUT, PCR and histology were used to detect H.pylori and PCR for cagA detection. TUNEL histochemistry was used to detect apoptosis. Immunohistochemical staining was used to detect PCNA, Fas/FasL, Bax and Bcl-2. Use ANOVA, Student's t-test in SPSS software as the statistic methods.Results The infection rate of H.pylori is 60.0%,and rate of cagA(+) is 90.47%. Expression of PCNA, Fas/FasL,Bax and Bcl-2 in H.pylori(V) specimens in atrophy group are higher than H.pylori(-) ones, however the apoptosis index is lower(/7<0.05).There is no difference of Hp(+) and Hp(-) specimens in atrophy with light or moderate dysplasia group(p>0.05). The expression of PCNA, Fas/FasL, Bax and Bcl-2 in gastric adenocarcinoma are higher but apoptosis index is lower than other lesions (p<0.01). The cagA geng has no effect on the expression of PCNA, apoptosis, Fas/FasL, Bax and Bcl-2(p>0.05).Conclusions Our results suggest that H.pylori infection can induce theincreasing of apoptosis and proliferation, and can increase the expression of Fas/FasL, Bax and decrease the expression of Bcl-2 in gastric epithelial cells. The increasing of apoptosis undergoes via Fas/FasL pathway. Gastric adenocarcinoma can express higher amount of PCNA, Fas, FasL, Bax and Bcl-2, which plays a great role in the formation of stomach cancer.
