Cognitive Function Of 71 Patients With Maintenance Hemodialysis And Characteristics Of TCM Syndromes

BackgroundThe number of maintenance hemodialysis patients are increasing due to the high morbidity of the chronic kidney disease,a research shows the annual incidence of hemodialysis patients in our country is 187 per 1 million. The main kidney replacement therapy including hemodialysis, peritoneal dialysis and kidney transplant.As the development of dialysis techniques,the patients survive longer than before, but also has many kinds of complications during the treatment, cognitive impairment is one of them. maintenance hemodialysis patients many risk factors for cognitive impairment, many studies pointed out maintenance hemodialysis patients have a higher morbidity than the general population, between 16%-38%. cognitive impairment impacts the living quality reduces therapy compliance, might even cause death of the patients.ObjectiveTo assess the clinical features of cognitive impairment in maintenance hemodialysis patients and its potential risk factors.Investigate the distribution differentiation of TCM syndrome in hemodialysis patientsMethodMontreal cognitive assessment was applied to measure the cognitive function of maintenance hemodialysis patients. The general condition and laboratory tests of the patients enrolled has also been recorded. Hamilton depression scale and hamil ton anxi ety scale was also used in the research. TCM syndrome was determined by the patient’s symptom,pulse manifestation and the coating on the tongue.Results71 maintenance hemodialysis patients was selected in the research.73% of the hemodialysis patients enrolled was found to have congnitive impairment, it shows that the decline in six aspects of the patients cognitive function all have significant difference (P<0.05). The age, anxiety score, and serum uric acid diffirences between the congnitive impairment group and the normal cognitive function group have statistical significance (P<0.05). montreal cognitive assessment score was positively correlated with serum creatinine and serum uric acid, and negatively correlated with age, anxiety score and depression score (P<0.05).The main TCM syndrome of the 71 maintenance hemodialysis patients is the yin-yang deficiency which is also the major TCM syndrome of the congnitive impairment group. The main TCM syndrome of the normal cognitive function group qi-yin deficiency.ConclusionMaintenance hemodialysis patients has a high risk to get cognitive impairment,they seems to have a decline in all aspects of the MoCA,including visuospatial, executiveability,naming,attention,language, delayed recall. The age, with anxiety and depression might be the potential risk factors of congnitive impairment in maintenance hemodialysis patients. Yin-yang deficiency is the main TCM syndrome of Maintenance hemodialysis patients, there is a diffidence of the main TCM syndrome between congnitive impairment group and the normal cognitive function group.
