Study On Syndrome-Treatment And Prescription-Drugs Of Lumbago Disease By Ancient And Modern Medical Records

Objective:This study relies on the expertise of TCM medical records study,modern computer technology and statistical analysis methods to integrate and analyze the collected ancient and modern TCM medical records of lumbago disease,systematically and multi-dimensionally explore the hidden relationships in the data,and explore the evolutionary trajectory and treatment laws of lumba-go disease,with the hope of providing some reference for clinical diagnosis and treatment of lumbago disease;in order to provide theoretical basis for future experimental research and clinical application,the network pharmacolo-gical analysis of core group drugs for lumbago disease is carried out to explore their relieve pain mechanism.Methods:1.Through the method of literature,the medical records of lumbago in ancient times were collected and sorted out.Taking the historical dynasties as the timeline,the authors comprehensively summarized and reviewed the names,classifications,etiology and pathogenesis,treatment principles and methods of lumbago,so as to systematically understand the development rules of this disease from an overall perspective and lay a literature foundation for future research on lumbago.2.We collected a wide range of ancient and modern TCM medical records of lumbago disease,and after screening for diagnosis,inclusion,and exclusion criteria,extract information such as medical record age,patient gender,patient age,onset season,Western medicine diagnosis,etiology,disease location syndrome elements,disease nature syndrome elements,symp-toms,drugs,formulas,methods of taking medication,and standardize the data.Through field and program design,the database of symptoms and drugs of Song-Jin-Yuan period,Ming-Qing period and modern times medical records was constructed.We reasonably applied SPSS26.0,Lanterns.0,VOSviewerl.6.18,R4.2.1,and R-Studio to analyze and mine the databases,and explored the internal syndrome and treatment laws of lumbago disease from different perspectives and levels.3.Using TCMSP,UniProt database,GeneCards,DisGeNET,TTD disease database,to screen the compounds and targets of core group drugs for treating lumbago.Construction of drug-effective compound-target network by Cytos-cape;STRING was used to construct target protein interaction network.BioGPS database was used to locate the potential targets in tissues and organs.DAVID was used for GO and KEGG enrichment analysis of potential targets.Results:Through the analysis of the statistical results,it is concluded that the incidence of lumbago is based on the basic deficiency with solid.Summarize the main points of syndrome differentiation and treatment such as pathologic-al basis,syndrome groups and syndrome types of lumbago,analyze the comm-only used drugs,drug pairs and prescriptions,treatment principles and methods,syndrome and treatment rules and evolution of lumbago,and summa-rize the clinical diagnosis and treatment ideas of lumbago disease,namely,"tonifying deficiency and dispelling evil,based on the five viscera syndrome differentiation,classify kidney deficiency as the core cause,analyze the pathogenic factors of cold dampness,damp heat,phlegm stasis,etc."Combined with the analysis of hidden structure,it expands the connotation of diagnosis and treatment of lumbago and supply a consult for the clinical treatment of lumbago.Conclusion:1.Most of the causes of low back pain are improper lifestyle,iatrogenic factors,special endowment status and pathological factors.2.The core disease location of low back pain is kidney meridians liver and spleen of which kidney is the core.In the course of its pathogenesis the disease changes are complex and deficiency,but always "kidney deficiency"and "invasion of external evil" these two core elements.3.The distribution of syndromes also showed certain differences with different historical periods,gender,age and onset seasons,indicating that the historical background and the construction of knowledge system framework had a great influence on the distribution of syndromes.4.Through the analysis and interpretation of hidden structure,it is concluded that the main syndroms of low back pain include some syndroms.5.Summarized the treatment principle is tonifying kidney and consolidating root,removing evil and clearing collaterals;The general treatment methods are dispelling cold and promoting dampness,clearing heat and removing dampness,promoting blood circulation and removing blood stasis,tonifying kidney(Yin and Yang)and strengthening waist.6.The core drug group(Angelica-Eutcha-Achyranthus)can have multi-component,multi-target,multi-organ,multi-channel tonifying kidney and strengthening waist,soothing tendons and relieving pain,and have synergistic effects from two aspects of symptoms and causes.
