| Objective: Nurses bear a high level of work responsibility and pressure,often facing trauma,death,and sudden public health events,which can easily increase the psychological burden of nurses and generate negative psychological emotions such as anxiety and depression.This not only affects their physical and mental health,but also reduces the quality of nursing work and medical services.The psychological burden of nurses in tertiary hospitals in China is becoming increasingly severe.Therefore,the mental health status of nurses in tertiary hospitals is particularly noteworthy.In addition,influenced by the global COVID-19 pandemic,medical staff have experienced severe anxiety and depression,especially the anxiety and depression problems of nursing staff are on the rise worldwide.Therefore,it is necessary to comprehensively understand the current situation and influencing factors of anxiety and depression among nurses in tertiary hospitals during the epidemic prevention period.Numerous studies had shown that anxiety and depression are the result of a combination of genetic,environmental,psychological,and social factors.However,existing research mostly focuses on the impact of a certain aspect on depression and anxiety,lacking a comprehensive analysis of the current situation and influencing factors of depression and anxiety among nurses in tertiary hospitals.Occupational burnout can increase nurses’ psychological stress and discomfort,thereby inducing or exacerbating symptoms of anxiety and depression.High job satisfaction can enhance increased confidence and happiness,alleviate psychological burden and sense of difficulty,and thus prevent or alleviate symptoms of anxiety and depression.Positive psychological intervention,as an emerging treatment method,can successfully improve people’s subjective consciousness and mental health,while also helping to alleviate depressive symptoms.A large number of studies had shown that positive psychological intervention can significantly improve the psychological status of cancer patients,elderly people,and college students.However,relevant research applied in the field of nursing is still in the development stage,and there is no unified intervention model,and there are differences in intervention results.With the rapid development of information technology,mobile healthcare has broken through time and space limitations and provided convenience for psychological intervention.Positive psychological factors are also important manifestations of individual cognition.According to Beck’s cognitive model of emotional disorders,negative cognitive biases and negative self-evaluation are susceptible factors leading to emotional disorders.Existing research mostly starts from a negative perspective,with few people paying attention to whether positive self-awareness can reduce the occurrence of negative psychology.Therefore,this study is based on Beck’s cognitive model of emotional disorders and the theory of positive psychology.From the general situation(demographic characteristics,occupation,department,monthly income,work hours,workplace violence,chronic diseases,etc.),positive psychological factors(self compassion,resilience,recovery experience)Comprehensively understand the anxiety and depression status and influencing factors of nurses in tertiary hospitals during the epidemic prevention period,including occupational related factors(occupational identity,occupational burnout,organizational support,job satisfaction),as well as individual behavioral and psychological characteristics factors(stress perception,sleep disorders,and fatigue);Further explore the mechanism of the influencing factors of nurses’ job burnout and job satisfaction,as well as the direct and indirect mechanisms of the influencing factors of depression and anxiety,and attempt to construct a risk prediction model to find feasible intervention methods,providing clues for positive psychological intervention plans for nurses’ mental health.Based on the actual investigation of the psychological health status and occupational burnout of nurses,a positive psychological intervention plan for the nursing population in tertiary hospitals was established and implemented using mobile medical means.The intervention effect was evaluated to improve the current situation of depression,anxiety,and occupational burnout among nurses,providing a scientific basis for comprehensively promoting the physical and mental health of nurses and ensuring the quality of medical services.Methods: This study selected nurses from one tertiary hospital in each city of Fushun,Liaoyang,Fuxin,and Tieling in Liaoning Province as the research subjects.A cross-sectional design was used to conduct a questionnaire survey on all randomly selected research subjects.The survey tools include: Generalized Anxiety Disorder Scale(GAD-7),Self Rating Depression Scale(PHQ-9),Fatigue Scale,Perceived Stress Perception Scale(PS-10),Connor Davidson Resilience Scale(CD-RISC),Recovery Experience Scale,Empathy Scale,Organizational Identity Scale,Organizational Support Scale,Job Burnout Scale,Minnesota Satisfaction Scale,and Simplified Coping Style Questionnaire.Two comprehensive tertiary hospitals were selected and the research subjects were randomly divided into an intervention group and a control group,and intervention studies were conducted.Based on mobile medical technology,a 6-week positive psychological intervention experiment was conducted on the research subjects of the intervention group through a mobile medical platform,while the control group did not take any measures.Use descriptive methods(mean,standard deviation,percentage)to describe the characteristics of the research objects.The correlation between research variables was analyzed using Pearson correlation analysis.Multiple stratified regression analysis was used to analyze the comprehensive influencing factors of nurses’ anxiety and depression;Using Model 6 of the SPSS macro program Process,establish a chain mediation model to explore the influencing factors and mechanisms of nurse burnout;Using R language to construct a risk prediction model;Compare the differences in scale scores between two groups at different time points in the intervention study using repeated measurement analysis of variance,and evaluate the intervention effect.Result: 1.A total of 1400 subjects were included in this study,all of whom were women,of which 763 were under 35 years old,accounting for 54.50% 637 people aged35,accounting for 45.50%。2.The average score of nurse anxiety is(6.38±5.30).Nurses with other marital status,bachelor’s degree or above,monthly income≤6000 yuan,and exposure to verbal and physical violence have higher levels of anxiety(P<0.05);The anxiety level of nurses is positively correlated with sleep disorders,fatigue,and stress perception;The anxiety level of nurses is negatively correlated with resilience,organizational support,professional identity,sympathy,and recovery experience(P<0.05).The results of a hierarchical regression model for anxiety show that organizational support and empathy negatively predict anxiety,while sleep and recovery experiences positively predict anxiety.3.The average score of depression among nurses is(8.16±6.39)Nurses aged 35,with other marital status,a medical department,a monthly income of less than 6000 yuan,and experiencing verbal and physical violence had higher levels of depression(P<0.05).The depression level of nurses is positively correlated with sleep disorders,fatigue,and stress perception;Nurses’ depression was negatively correlated with resilience,organizational support,professional identity,sympathy,and recovery experience(P<0.05).The results of a hierarchical regression model for depression showed that resilience negatively predicted depression,while sleep disorders and stress perception positively predicted depression.4.The total score of nurse burnout is(11.46 ± 5.91)Nurses aged 35,with a bachelor’s degree or above had a higher total score of job burnout(P<0.05).Nurses with a monthly income of less than 6000 yuan,and exposure to verbal and physical violence had higher total scores of occupational burnout(P<0.05).5.The internal job satisfaction of nurses is(42.95±10.28)points,and the external job satisfaction is(28.05 ± 7.20)points Nurses aged 35 had higher internal job satisfaction(P<0.05).Nurses with a supervisory nurse or above,a medical department,a monthly income of less than 6000 yuan,a weekly length of more than 40 hours,and exposure to verbal or physical violence had lower overall job satisfaction scores(P<0.05).6.There is a negative correlation between job burnout and satisfaction(P<0.001).Nurse burnout is positively correlated with anxiety,depression,sleep disorders,fatigue,and stress perception;Nurse job burnout is negatively correlated with resilience,organizational support,professional identity,sympathy,recovery experience,and stress response(P<0.05).The results of a hierarchical regression model for nurses’ occupational burnout show that occupational identity and organizational support,emotional stress response,and sympathy negatively predict occupational burnout,while sleep disorders,stress perception,and fatigue positively predict occupational burnout.7.Restoration experience can significantly negatively predict job burnout(β=-0.278),positive prediction of job satisfaction(β=0.434),self compassion can significantly positively predict job satisfaction(β=0.012),significantly negative prediction of job burnout(β=-0.032),job satisfaction can significantly negatively predict job burnout(β=-0.52).Self sympathy and job satisfaction play a chain mediating role in the negative effect of recovery experience on job burnout.8.The AUC value of the depression prediction model is 0.88,the sensitivity value is0.91 and the specificity value is 0.58.The AUC value of the anxiety prediction model is0.92,the sensitivity value is 0.76 and the specificity value is 0.92.9.A total of 149 research subjects were included in the intervention study.As the intervention progressed,the anxiety level of the intervention group nurses significantly decreased.The initial anxiety score of the experimental group was(6.63±4.27)points,and the final score decreased to(5.47±3.68)points;However,there was no significant change in the anxiety level of nurses in the control group.The depression level of the intervention group nurses significantly decreased,with an anxiety score of(9.05±5.30)in the initial experimental group and(6.83±4.45)in the final stage;In the control group,the level of depression among nurses increased during the mid intervention period,but decreased at the end of the intervention period,with no significant change from the initial stage.Conclusion: 1.The level of anxiety and depression in the nurse population is relatively high.Nurses with other marital status,lower monthly income levels,longer working hours,and experiencing verbal and physical violence have higher levels of anxiety and depression.Nurses with sleep disorders,fatigue,and feelings of stress are more likely to experience anxiety or depression.On the contrary,if nurses have higher resilience,organizational support,professional identity,sympathy,and recovery experience,their levels of anxiety and depression will decrease.2.Nurses generally experience job burnout,with low job satisfaction.Nurses who experience anxiety,depression,sleep disorders,fatigue,and stress have a higher level of burnout.On the contrary,if the resilience,organizational support,professional identity,sympathy,recovery experience,and stress response of nurses are higher,their fatigue level is lower.The restorative experience can significantly alleviate an individual’s level of job burnout through the chain mediating effect of self compassion and job satisfaction.Individual psychological and behavioral characteristics can be comprehensively influenced by positive psychological resources and professional factors on the psychological status of nurses.In the mobile medical mode,providing nurses with more positive psychological intervention is beneficial for reducing their anxiety and depression levels,alleviating their mental stress state,and also preventing or reducing their level of fatigue. |
