Study On“ling-gui Ba-fa”Bee Venom Acupuncture Formula In The Treatment Of Chronic Fatigue Syndrome

ObjectiveChronic fatigue syndrome(CFS)also called Myalgic Encephalomyelitis(ME)is a common illness in the modern world characterized by profound tiredness and extreme fatigue for at least six months regardless of bed rest.It has a severe and long-term effect in on physical and mental health.This study summarized and analyzed the clinical application of traditional Chinese medicine therapy in the treatment of CFS.Through literature research,the feasibility of Ling-gui Ba-fa bee venom acupuncture for the treatment of CFS was analyzed.A prospective clinical controlled study was used to explore the clinical efficacy of Ling-gui Ba-fa bee venom acupuncture in the treatment of CFS,in order to provide new treatment strategies for the treatment of this disease,and at the same time prepare a theoretical basis and clinical evaluation for the treatment of CFS for Ling-gui Ba-fa of bee venom acupuncture.Methods1.Literature ReviewIn this research,the clinical application of traditional Chinese medicine therapy in the treatment of CFS in the past decade was studied,and large literature local and international databases were reviewed by electronic search.The three major Chinese databases included CNKI,WF and VIP,and the two major English literature databases were PubMed and Embase.A 10-year publication of literature which was from January 1,2013 to December 31,2022 related to the treatment of CFS by traditional Chinese medicine therapy was researched.The literature of the clinical application of the Ling-gui Ba-fa in the past decade was studied,and the large literature databases locally and globally were searched through electronic retrieval.In this study,three major Chinese databases were reviewed,including CNKI,WF and VIP.And two English literature databases were based on PubMed and Embase.A 10-year publication of literature which was from January 1,2013 to December 31,2022 related to the literature related to the clinical application of the ling-gui Ba-fa of spirit turtle published last decade,which was from January 1,2013 to December 31,2022,were researched.According to the eleventh revision of the International Classification of Diseases(ICD-11)Chinese edition,the literature was analyzed and summarized,and the frequency of each disease in the clinical literature of Ling-gui Ba-fa was counted.The literature of the clinical application of Bee venom acupuncture was studied.2.Clinical ResearchThis study is prospective randomized controlled trial.According to the inclusion criteria and exclusion criteria,90 patients who with spleen-kidney yang diciency constitution and chronic fatigue syndrome were recruited from the Department of Pre-existing Disease in Yangjiang Hospital of Traditional Chinese Medicine,affiliated with Guangzhou University of Chinese Medicine.The cases were studied from January 2022 to December 2022.90 patients were randomly selected and grouped into three groups by using Microsoft Excel:the Ling-gui Ba-fa Bee Venom Acupuncture Group,the Ordinary Bee Venom Acupuncture Group,and the Acupuncture Group,30 patients in each group were studied.The Ling-gui Ba-fa Bee Venom Acupuncture Group(The Treatment Group)used Ling-gui Ba-fa bee venom acupuncture to activate the acupoint(Eight-confluence-points)and basic acupoints bee venom acupuncture.According to the treatment time of the patients,use Ling-gui Ba-fa bee venom acupuncture to pick the acupoint,and then the bee venom acupuncture to sting the basic acupoints which are bilateral Zusanli(ST36),bilateral Shenshu(BL23),and Qihai(CV6),and Guanyuan(CV4).During the treatment,the basic acupoints were divided into three groups which were bilateral Zusanli as the first pair,bitlateral Shenshu as the second pair,Qihai and Guanyuan as the third pair.The bee venom acupuncture treatment was carried out in turn.The Ordinary Bee Venom Acupuncture Group(The Control Group 1)used comfort acupoints bee venom acupuncture and basic acupoints bee venom acupuncture.The comfort points which calculated based on the Ling-gui Ba-fa points were the replacement points of Ling-gui Ba-fa points,and the comfort points are non-meridian and non-acupoint.Basic acupoints were the same as the Linggui Bafa Bee Venom Acupuncture Group.The Acupuncture Group(The Control Group 2)was treated with Ling-gui Ba-fa acupuncture to activate the acupoint and simple acupuncture.The basic acupoints of simple acupuncture which are bilateral Zusanli(ST36),bilateral Shenshu(BL23),Qihai(CV6)and Guanyuan(CV4)were taken.Each participant received two 10-day courses of treatment.The participant got the treatment on alternate days,and there was a 2-day break between the two courses.The patient was interviewed after each course.The efficacy evaluation indicators of this study were Fatigue scale-14(FS-14)score,Visual analogue scale(VAS)pain score,Self-rating Anxiety scale(SAS)score,and Self-rating Depression scale(SDS)score,which were compared before and after treatment.In terms of clinical efficacy,efficacy evaluation was carried out after the course of treatment.Finally,SPSS 23.0 statistical software was used to perform statistical analysis on the study data.Results1.Literature ReviewA total 614 articles were included and classified into the clinical application of traditional Chinese medicine therapy in the treatment of CFS.237 articles on acupuncture or combined with other traditional Chinese medicine;194 articles on oral Chinese medicines or combined with other traditional Chinese medicine;163 articles on moxibustion or combined with other traditional Chinese medicine;74 articles on massage or combined with other traditional Chinese medicine;39 articles on acupoint sticking or combined with other traditional Chinese medicine;36 articles on cupping therapy or combined with other traditional Chinese medicine;22 articles on traditional Chinese medicine exercises or combined with other traditional Chinese medicine;18 articles on catgut embedding or combined with other traditional Chinese medicine;15 articles on scraping therapy or combined with other traditional Chinese medicine;14 articles on music therapy or combined with other traditional Chinese medicine;7 articles on medicated bath therapy or combined with other traditional Chinese medicine and 7articles on other therapies.A total 179 articles were included and classified into the disease spectrum of Ling-gui Ba-fa.This study obtained 14 types of disease systems:48 nervous system disorders;25 mental and behavioral disorders;23 skeletal system and connective tissue disorders;23 genitourinary disorders;11 endocrine,Nutritional and metabolic disorders;9 tumor disorders;4 respiratory system;4 Skin and subcutaneous tissue disorders;2 Circulatory disorders;2 Infectious and parasitic diseases;2 Eye and appendage disorders;2 Injury,poisoning and external causes;2 Ear,Nose and Throat disorders;1 Oral system disorders.According to the research of Shiyun Chen(2012),Changjing Xu(2016),Huiming Chen(2019),bee venom acupuncture were using in 14 types of disease system,78 categories of diseases.2.Clinical ResearchThe data of 86 patients with Chronic Fatigue Syndrome who met the inclusion criteria were collected.Including 29 patients in the Ling-gui Ba-fa Bee Venom Acupuncture Group,29 patients in the Ordinary Bee Venom Acupuncture Group,and 28 patients in the Acupuncture Group.According to the data of gender,age,education,occupation,marital status and place of residence of the three groups,there was no significant difference(P>0.05)after statistical analysis,which was comparable.2.1 Comparison of clinical efficacyAfter the first course of treatment,1 patient is cured,6 patients were found obviously improved,16 were patients found improved,and 6 patients were found invalid in the Ling-gui Ba-fa Bee Venom Acupuncture Group.For the Ordinary Bee Venom Acupuncture Group,no patient is cured,2 patients were found obviously improved,1 patient was found improved,and 16 patients were found invalid.For the Acupuncture Group,no patient is cured,6 patients were found obviously improved,11 patients were found improved,and 11 patients were found invalid.There were differences between the three groups(P=0.019<0.05),which is statistically significant.After the second course of treatment,3 patients are cured,13 patients were found obviously improved,10 patients were found improved,and 3 patients were found invalid in the Ling-gui Ba-fa Bee Venom Acupuncture Group.For the Ordinary Bee Venom Acupuncture Group,1 patient is cured,8 patients were found obviously improved,8 patients were found improved,and 12 patients were found invalid.For the Acupuncture Group,1 patient is cured,7 patients were found obviously improved,12 patients were found improved,and 8 patients were found invalid.There were differences between the three groups(P=0.019<0.05),which is statistically significant.After the first course of treatment,the overall response rate of the Ling-gui Ba-fa Bee Venom Acupuncture Group was 79.3%,and the marked response rate was 24.1%.For the Ordinary Bee Venom Acupuncture Group,the overall response rate was 44.8%,and the marked response rate was 6.9%.For the Acupuncture Group,the overall response rate was 60.7%,and the marked response rate was 21.4%.The differences in the overall response rate among the 3 groups after the first course of treatment were statistically significant,which is(P<0.05),while the marked response rates were not different(P>0.05).After the second course of treatment,the overall response rate of the Ling-gui Ba-fa Bee Acupuncture Group was 89.7%,and the marked response rate was 55.1%.For the Ordinary Bee Venom Acupuncture Group,the overall response rate was 58.6%,and the marked response rate was 31%.For the Acupuncture Group,the overall response rate was 71.4%,and the marked response rate was 28.6%.There was a statistically significant difference in the overall response rate among the 3 groups after the second course of treatment,which is(P<0.05),while the differences in the marked response rates were not different(P>0.05).2.2 Intre-group comparison of pre-and post-treatment scale scoresAfter treatment,FS-14 scores,SAS scores and SDS scores in all three groups decrease significantly after the first course,and decreased more significantly after the second course(P<0.05).The results showed that the three groups of patients could effectively improve the fatigue,depression and anxiety of patients after treatment,and the effect was obvious.2.3 Comparison between groups after treatmentFor the FS-14scores,the improvement of the groups of patients was statically significant(P<0.05)after the first course of treatment.The scores of the Ling-gui Ba-fa Bee Venom Acupuncture Group and the Ordinary Bee Venom Acupuncture Group are significantly different(P<0.01).There are differences between the score of the Ling-gui Ba-fa Bee Venom Acupuncture Group and the Acupuncture Group(P<0.005),while no difference between the Ordinary Bee Venom Acupuncture Group and the Acupuncture Group(P>0.05).Basisx±s,the decrease in the FS-14 score of the Ling-gui Ba-fa Bee Venom Acupuncture Group was significantly better than that of the Acupuncture Group,and the Acupuncture Group was better than the Ordinary Bee Venom Acupuncture Group.After the second course of treatment,the FS-14 Scores of three groups of patients are with statistically significant differences(P<0.01).The difference between the Ling-gui Ba-fa Bee Venom Acupuncture Group and the Ordinary Bee Venom Acupuncture Group is statistically significant(P<0.01),the difference was not statistically significant between the Ling-gui Ba-fa Bee Venom Acupuncture Group and the Acupuncture Group(P>0.05),but the difference was significant between the Ordinary Bee Venom Acupuncture Group and the Acupuncture Group(P<0.005).It showed that the Ling-gui Ba-fa Bee Venom Acupuncture Group and the Acupuncture Group improved fatigue symptoms better than the Ordinary Bee Venom Acupuncture Group,according to(P25,P75),Ling-gui Ba-fa Bee Venom Acupuncture Group better than the Acupuncture Group.For the VAS pain score,after the first course of treatment and the second course of treatment,the VAS pain score comparison of the three groups was not statistically different(P>0.05).However,according to(x±s P25,P75),the reduction in the VAS pain score in the Ling-gui Ba-fa Bee Venom Acupuncture Group was significantly better than that of the Ordinary Bee Venom Acupuncture Group and the Acupuncture Group,and the Acupuncture Group was better than the Ordinary Bee Venom Acupuncture Group.For the SAS score,After the first course of treatment and the second course of treatment,the SAS score of the Ling-gui Ba-fa Bee Venom Acupuncture Group was compared with the Ordinary Bee Venom Acupuncture Group and the Acupuncture Group,and the difference was not statistically significant(P>0.05).According to x± s(P25,P75),the reduction of SAS score in the Ling-gui Ba-fa Bee Venom Acupuncture Group was significantly better than that of the the Ordinary Bee Venom Acupuncture Group and the Acupuncture Group,and the Acupuncture Group was better than the Ordinary Bee Venom Acupuncture Group.After the first course of treatment,there was no statistically significant difference in scores(P>0.05)when the SDS scores were compared.When the Ling-gui Ba-fa Bee Venom Acupuncture Group and between other two Group are not statistically significant(P>0.05).Basis x±s and(P25,P75),The decrease in the SDS score in the Ling-gui Ba-fa Bee Venom Acupuncture Group was significantly better than that of the Ordinary Bee Venom Acupuncture Group and the Acupuncture Group,and the Acupuncture Group was better than the Ordinary Bee Venom Acupuncture Group.After the second course of treatment,the improvement of the groups of patients was statistically significant(P<0.05).There was a statistically significant difference between the Ling-gui Ba-fa Bee Venom Acupuncture Group and the Ordinary Bee Venom Acupuncture Group(P<0.01),The differences between the Ling-gui Ba-fa Bee Venom Acupuncture Group and the Acupuncture Group,and the Ordinary Bee Venom Acupuncture Group and the Acupuncture Group were not statistically significant(P>0.05).ConclusionLiterature research shows that traditional Chinese medicine therapy has a significant therapeutic effect on Chroin fatigue syndrome,and the Linggui Eight Methods and Bee Needle can be used to treat various clinical symptoms of chronic fatigue syndrome.Clinical Research showed that Ling-gui Ba-fa bee venom acupuncture,ordinary bee venom acupuncture,and acupuncture can all improve fatigue,pain,anxiety,and depression in patients with Chronic Fatigue Syndrome to varying degrees.According to the analysis of FS-14 scores,VAS pain scores,SAS scores and SDS scores,Ling-gui Ba-fa Bee Venom Acupuncture group found more significantly drop in those scores that those in the Ordinary Bee Venom Acupuncture Group and the Acupuncture group.In terms of overall efficacy,Ling-gui Ba-fa bee venom acupuncture were the best,and the acupuncture was slightly better than the ordinary bee venom acupuncture.It showed that the treatment of CFS with Ling-gui Ba-fa bee venom acupuncture was more conducive to the recovery of patients than the other two needles,and the qauality of life of patients could be improved.Ling-gui Ba-fa Bee Venom Acupuncture Group reduced Fatigue Scale-14(FS-14)scores,Visual Analogue Scale(VAS)pain scores,Self-rating Anxiety Scale(SAS)scores and Self-rating Depression scale(SDS)scores.It is more significant than the other two groups,and the overall curative effect is better,which shows that Ling-gui Ba-fa bee venom acupuncture is more conducive to the rehabilitation of CFS patients and improves the quality of life of CFS patients.
