| As the “gatekeeper” of residents’ health,general practitioners shoulder the heavy responsibility of the first line of defense for the whole social medical security system,especially in the outbreak of New Coronavirus in recent years,playing an important role in medical protection.In order to speed up the implementation of the “graded diagnosis and treatment” policy and form an interactive mode of medical information transmission with large hospitals,the realization of the service value of general practitioners is the key link.However,the current slow professional growth of general practitioners limits the realization of their service value.No matter what kind of environment they are in,general practitioners have universal characteristics of career growth.General practitioners are comprehensive medical talents who provide medical services such as preventive health care,referral and chronic disease management.In the face of the busy medical work,it is necessary to develop a segmented goal management plan to promote the career growth of general practitioners.The greatest feature of general practitioners is that they consider the possible causes of diseases with systematic scientific ideas,which is not limited to the logic of “treating foot pain and treating head pain”.Therefore,scientific thinking should become their inertia consciousness in analyzing problems.Due to the increasing aging of the social population,the state has begun to pay more attention to the development of general practitioners,which will put forward higher requirements for their innovative spirit.Scientific thinking and innovative spirit have become the most valuable career growth opportunity given to general practitioners in this era.The biggest difference between general practitioners and specialists is that specialists manage diseases while general practitioners manage people.General practitioners are health managers in the long-term life-cycle responsible care of physical and mental health from birth to death,which tests their internal cultivation and humanistic spirit.In order to have a more comprehensive and in-depth understanding of the real cause of the patient’s illness,general practitioners should not only have the basic professional development ability,but also pay more attention to the development of communication skills and coordination ability.Therefore,the progress of career goals,the development of professional ability and the emotional input of the post will be the key elements of the career growth of general practitioners.In the context of the slow development of the whole grass-roots medical and health services,the career growth of general practitioners is affected by many complex factors.In addition to the training mechanism and incentive mechanism at the organizational level,the degree of professional identity and the pursuit of self-achievement at the psychological level of general practitioners,as well as the sense of self-efficacy reflected in the face of complex problems,may become the key factors affecting their career growth.This dissertation studies the impact of general practitioners’ career growth on their service value from the perspective of constructing the dimension of general practitioners’ career growth,analyzing the factors of general practitioners’ career growth,and exploring the mechanism between general practitioners’ career growth and their service value.First of all,the program grounded theory method is used to construct the career growth dimension of general practitioners,and exploratory factor analysis and confirmatory factor analysis are used to test the scientificity of the theoretical model,which not only enriches the research theory in the field of career growth,but also provides important theoretical support for the career growth of general practitioners.Secondly,aiming at the problem of slow career growth of general practitioners,this dissertation breaks through the previous regression analysis method to emphasize the net effect of single factor on the career growth of general practitioners,and considers that their career growth may be affected by multiple factors.Therefore,from the perspective of set theory,this dissertation analyzes the causal complexity between conditions,uses configuration thinking to build a linkage matching model of psychological and organizational factors,explores the pathways to generate high professional growth of general practitioners,and analyzes the mutual substitution between conditions.Finally,based on the combination of self-determination theory,social exchange theory,service value theory and career development theory,this dissertation constructs a theoretical model of the impact of general practitioners’ career growth on their service value,which takes general practitioners’ career growth as the independent variable,general practitioners’ service value as the dependent variable,work input as the intermediary variable,and organizational support perception and perceived obligation as the regulatory variables,through hierarchical regression analysis and structural equation model to verify the specific mechanism between them.This not only completes the preliminary research on the impact of career growth on its service value,but also provides important theoretical reference value for the research on career growth and service value in the field of medicine and management.(1)The career growth of general practitioners constructed according to qualitative research methods mainly includes three dimensions: career goal progress,career ability development and post emotional investment.The test results show that the three assumed dimensions can better converge to the higher-level concept of career growth.At the same time,the model also passed the test of reliability and discriminant validity.Therefore,it can be judged that the structural design of the model is more reasonable.The development of general practitioners’ career growth scale not only provides a certain reference value for the government and community hospitals to make scientific management decisions and plans,but also provides a theoretical basis for general practitioners to clarify the content of their own career growth.(2)Through QCA configuration analysis,it shows that there are three pathways affecting the high career growth of general practitioners,and the optimal path is obtained,namely A2(i.e.professional identity*achievement motivation*self-efficacy*incentive mechanism).It is found that the generation of high career growth of general practitioners is more important to be affected by the linkage and matching between psychological and organizational factors.This conclusion expands the antecedent variables of general practitioners’ career growth,and provides an important practical basis for improving the quality and efficiency of general practitioners’ career growth.Hierarchical regression analysis and structural equation method were used to test the theoretical model of the impact of general practitioners’ career growth on their service value.The results show that in the main effect and intermediary effect test of the hierarchical regression analysis model,the career growth of general practitioners significantly positively predicts work input and service value,while work input significantly positively predicts service value,and work input plays a complete intermediary role in the process of professional ability development affecting service value,and work input plays a partial intermediary role in the other two relationships.In the robustness test of the research conclusion,it is found that the research conclusion is consistent with the hypothesis.The above conclusions play an important role in realizing the service value of general practitioners,thus increasing the trust of patients and affecting their medical behavior. |
