| Objective:Through literature study,TCM etiology,therapeutic principle and method,treatment principles,syndrome differentiation and treatment of untoward effect in chemotherapy for gastric cancer were summarized.To observe the clinical efficacy of using the method of nourishing Qi and invigorating spleen in the chemotherapy of gastric cancer patients combined with the instructor’s experience prescription.Analysing clinical observation results and Mechanism of action to confirm the dominant position of traditional Chinese medicine in treating gastric cancer,and provide reference value for optimal scheme.In this study,TCM Jianpi Decoction was used to treat adverse reactions of chemotherapy in the treatment of gastric cancer.The treatment principle of Jianlian Doction was used to replenish the qi,invigorate the spleen and stomach,dispel evil spirits and strengthen the body,and strengthen the foundation by nourishing the yuan.In the process of the development of tumor diseases,TCM believes that the patients had deficiency of qi and weakness of spleen and stomach,which were aggravated by a malignant tumor—a kind of consumptive disease.Thus,the treatment was based on the TCM Yiqi Jianpi Decoction to reduce the adverse reaction of gastric cancer after chemotherapy,reduce the side effects of chemotherapy,and improve the body’s internal disease resistance and recovery ability,which was of great significance for clinical treatment of tumor diseases.Methods:A total of 120 patients with gastric cancer who met the inclusion criteria for chemotherapy were collected,including 60 patients in the treatment group receiving Yiqi Jianpi Decoction combined with DC chemotherapy,and other 60 patients in the control group only receiving DC chemotherapy and anti emetic treatment.Clinical symptoms,acute and subacute toxic reaction,immune indexes,serological indexes,tumor indexes and quality of life rating scale were observed before and after treatment in the two groups,and the difference of efficacy between the two groups was compared.Results:1.Baseline comparison:There were 60 cases in the treatment group and the control group respectively.Based on the analysis of basic index data,there was no significant difference between the two groups before treatment(P>0.05).When analyzing the clinical stage distribution data,x2 test showed P>0.05,which means no statistical difference.The single symptom and physical sign of traditional Chinese medicine was close(P>0.05).There was little difference between the groups,which met the test requirements.2.Improvement of TCM symptoms:Statistical analysis of the data of the two groups before and after treatment showed that within-group comparisons after treatment,the improvement of all single TCM symptoms in the treatment group was more significant(P<0.01).It showed that the symptoms of patients are alleviated after the intervention of chemotherapy and Chinese medicine,and the intervention of Chinese medicine can improve the symptoms.And in control group,patients got significant improvements only in the reduced appetite,postcibalabdominal distension,and nausea and vomiting after treatment(P<0.05).It showed other symptoms such as fatigue and lack of strength,irregular bowel movement,vapidity,continuous and dull abdominal pain,and sallow complexion didn’t get significant improvement after symptomatic treatment towards liver protection,stomach protection and antiemetic.Compared with the control group,after treatment,the three symptoms of the treatment group were significantly improved(P<0.01),which hinted that the control group only can improve the symptoms of reduced appetite,postcibalabdominal distension,and nausea and vomiting for middle-late gastric cancer chemotherapy patients,and after TCM intervention in treatment group,not only can improve the symptoms of reduced appetite,postcibalabdominal distension,and nausea and vomiting,but also can improve other symptoms fatigue and lack of strength,irregular bowel movement,vapidity,continuous and dull abdominal pain,and sallow complexion didn’t get significant improvement after symptomatic treatment towards liver protection,stomach protection and antiemetic,which is much better than the control group.3.Total syndrome scores:Statistical analysis of patients’ syndrome scores showed that after treatment,both groups had significantly improved statistical results of scores,which were significantly different from those before treatment(P<0.01).Compared with the control group,the improvement of the treatment group was more obvious,and the difference between the two groups was very significant(P<0.01).4.Comparison of adverse reaction:This research showed that the adverse reaction of chemotherapy mainly include anemia,leukopenia,thrombocytopenia,nausea,vomiting,diarrhea,peripheral paresthesia,but were not serious,Ⅲor IV degree of toxicity reaction is rare.Compared with the control group,the reaction of leukopenia,nausea and vomiting in the treatment group was less severe,and there was statistical significance(P<0.05).In the treatment group,the incidence of 0 degree and Idegree of diarrhea was significant lessthan the control group,although without statistical significance,there was the tendency to improve the symptoms of diarrhea.There was no statistical significance in anemia,thrombocytopenia and neuroparesthesia between the two group(P>0.05).It indicated that Chinese medicine intervention after chemotherapy for gastric cancer patients can effectively alleviate leukopenia,nausea and vomiting,and has a tendency to improve diarrhea symptoms.5.Immune level comparison:Before treatment,the comparison between groups and the comparison of immune indexes was comparable(P>0.05).After treatment,the immune indexes in the treatment group were significantly improved(P<0.01).But there was no significant difference in the control group(P>0.05).After treatment,CD3+and CD4+in the treatment group were significantly higher than those in the control group(P<0.01),CD8+was significantly lower than those in the control group((P<0.01),and CD4+/CD8+was significantly higher than those in the control group(P<0.01).6.Comparison of serum indexes:Compared with before-treatment,IFN-γ in the treatment group was significantly increased(P<0.01).While there was no statistical significance in the control group.TNF-α in the treatment group was significantly decreased(P<0.01),while that in the control group was increased(P<0.01).After treatment,it was found that Yiqi Jianpi formula increased IFN-γ and decreased TNF-α,the differences between the two groups were statistically significant(P<0.01).7.Comparison of tumor markers:Before treatment,the data of the two groups were similar(P>0.05).Before and after treatment,there were significant differences in all tumor indexes in the treatment group(P<0.05).The levels of CEA,CA125 and CA199 in the control group were lower than those before treatment(P<0.05)compare among groups,the CEA and CA724 in the treatment group were much better than those in the control group(P<0.05).8.Comparison of quality of life scores:Before treatment,quality of life scores were close(P>0.05).After treatment,the quality of life scores in the two groups were higher than those before treatment(P<0.01)and the difference was significant in the treatment group(P<0.01).9.Comparison of efficacy:The effective rate of the treatment group was 85.00%,and the effective rate of the control group was 48.33%,and the difference between the two groups was statistically significant(P<0.05).Conclusion:1.The clinical observation results showed that gastric cancer chemotherapy combined with the use of TMC Yiqi Jianpi decoction can not only improve immune indexes,serological indexes and tumor indexes,but also reduce the acute and subacute toxic reactions of anti-tumor drugs,relieve TCM syndromes,improve the quality of life of patients,prolong the life,reduce the toxic and side effects of radiotherapy and chemotherapy,improve the completion rate of radiotherapy and chemotherapy,and promote the postoperative rehabilitation of tumor patients and other functions.Early intervention of traditional Chinese medicine in patients with gastric cancer undergoing chemotherapy can prevent the disease in advance,so as to reduce the pain of patients and enhance their confidence to resist the disease,fully reflecting the idea of preventive treatment of disease,which has important clinical application value,outstanding efficacy and high safety,and is worthy of promotion. |
