The First Part Clinical study on chronic tonsillitis treated by TCM Cauterization Purpose:To observe the clinical efficacy of TCM Cauterization method in many hospitals throughout the country,and to explore the feasibility of the application of TCM Cauterization method in clinical popularization..Material and method:Chose 148 patients with chronic tonsillitis in accordance with the conditions of admissionin at“State Key specialist in traditional Chinese Medicine”,the NHO of TCM.Treatment by TCM Cauterization,every 3 days was treated 1 times and 10 times for a course of treatment.The changes of symptoms and signs before and after treatment were compared,and the clinical curative effect,TCM syndrome score and the change of tonsillar hypertrophy were analyzed.Results: 1.The curative effect of TCM cauterization was obvious,56 cases were cured,55 cases were markedly effective,28 cases were effective,9 cases were invalid,the recovery rate was 75%,and the total effective rate was 93.92%.2.After treatment,the clinical symptoms and signs of the patients were significantly lower than those before treatment,and the difference was statistically significant(P < 0.05).3.The clinical symptoms of patients with pharyngeal obstruction,irritating cough,throat discomfort improved significantly;tonsil crypt became smaller and shorter,the opening is clearly visible,reduce pus embolus retention,reduce recurrence times.Conclusion:TCM cauterization on the premise of preserving tonsillar tissue,can effectively alleviate the patients with pharyngeal foreign body sensation,irritating cough,throat discomfort and other symptoms,reduce congestion and reduce the size of tonsils,tonsil pus embolus residual,clinical curative effect,the method is simple,easy to operate,can be repeated,is worthy of clinical application.The Second Part Experimental study on the effect of TCM Cauterization on the expression of cytokines in tonsil tissue of chronic tonsillitis Purpose:To observe the effect of TCM Cauterization on the expression of inflammatory factors in tonsillar tissue of patients with chronic tonsillitis.To discuss the mechanism of anti inflammation in patients with chronic tonsillitis by TCM Cauterization.Material and method : In patients with chronic tonsillitis treated by TCM Cauterization,random number method was used to select 10 patients randomly.It was taken some tonsillar tissues after 5 times of TCM cauterization and 5 times of before treatments.As a group of pre burn group(10 people)and branded group(10 people),10 tonsil physiological hypertrophy normal people tonsil tissues were selected as healthy control group(control group,10 people).The contents of IL-4,IL-5,IL-10,IFN-γ,TGF-β in tonsil tissues were detected by ELASA.Results: 1.Pre branding group patients’ hypertrophic tonsillars were accompanied by chronic tonsillar hyperemia,Cicatricial,extruded recess with suppository overflow.Patients have different degrees of foreign body sensation in the pharynx,halitosis and mechanical obstruction.After treatment,the size of the tonsil was significantly reduced,and the extruded recess without suppository was overflowing,and the symptoms of the patients were obviously improved.2.The contents of IL-4,IL-5,IL-10,IFN-γ andTGF-β in the tonsillar tissues of each group were detected:Compared with the blank control group,the contents of IL-4,IL-5,IL-10,IFN-γ and TGF-β in the tonsillar tissues of the other groups were significantly higher than those in the blank groups,and there were statistical differences(P < 0.01).Compared with pre-branded group,the contents of IL-4,IL-5,IL-10,IFN-γ,TGF-βin tonsil tissue were decreased in different degrees,and the difference was statistically significant(P < 0.01 or 0.05)..Conclusion: TCM Cauterization may have Anti-inflammatory effects on chronic tonsillitis,and this anti-inflammatory effect may be achieved by inhibiting the expression level of IL-4,IL-5,IL-10,IFN-γ,TGF-β and other cell factors in the tonsillar tissue.The Third Part Experimental study of the effect of TCM Cauterization on the TLRs-NF-κB signal pathway in tonsil tissue of chronic tonsillitis Purpose:Detect the express level of TLR1,TLR2,TLR6,MyD88,IRAK and NF-kappa B in tonsillar tissues of patients with chronic tonsillitis before and after the treatment of TCM Cauterization.To discuss the effect of TCM Cauterization on the TLRs-NF-kappa B signaling pathway in tonsillar tissue of chronic tonsillitis.Material and method : In patients with chronic tonsillitis treated by TCM Cauterization,random number method was used to select 10 patients randomly.It was taken some tonsillar tissues after 5 times of TCM cauterization and 5 times of before treatments.As a group of pre burn group(10 people)and branded group(10 people),10 tonsil physiological hypertrophy normal people tonsil tissues were selected as healthy control group(10 people).The expression level of TLR1、TLR2、TLR6、MyD88、IRAK、NF-κB protein in the tonsil tissues of each group was observed by Western-blot and Immunohistochemical method.The expression of TLR1、TLR2、TLR6、MyD88、IRAK、NF-κB in tonsillar tissue was observed by immunohistochemical method.The expression level and situation of TLR1-LR2 with double positive、TLR2-TLR6 with double positivein the nasal mucosa of rats were detected by double immunofluorescence.Results: 1.Compared with the healthy control group,the expression of TLR1、TLR2、TLR6、MyD88、IRAK、NF-κB in the tonsil tissues of other groups increased significantly,with statistical difference P < 0.01).Compared with the pre branding group,the expression of TLR1、TLR2、TLR6、MyD88、IRAK、NF-κB in the tonsil tissue of the post branded group was significantly down,with a statistically significant difference(P < 0.01).2.Compared with the blank control group,the TLR1-LR2 with double positivein and TLR2-TLR6 with double positivein expression in the tonsillar tissues of the other groups were significantly up-regulated,with statistical difference(P < 0.01).Compared with the pre-branding group,the expression of TLR1-LR2 with double positivein and TLR2-TLR6 with double positivein in the tonsil tissue of the post branded group was significantly down,with a statistically significant difference(P < 0.01).Conclusion: 1.The inflammatory reaction of chronic tonsillitis may be mediated by the TLR2-MyD88-NF-κB signaling pathway,and the activation of this pathway leads to the increase of the synthesis and secretion of inflammatory cytokines.2.TCM Cauterization has inhibitory effect on the TLR2-MyD88-NF-κB signaling pathway,and the anti-inflammatory mechanism of TCM Cauterization is closely related to the inhibition of activation of TLR2-MyD88-NF-κB pathway... |
