| The main pathological features of chronic atrophic gastritis(CAG)are chronic inflammation which is caused by atrophy of glands,thickening of muscular layer and thinning of mucosal layer.There are many factors influencing the occurrence and development of CAG,such as age,heredity,diet and rHP infection.Because CAG does not have typical clinical manifestations and the condition is repeated,it is difficult to cure thoroughly.The main symptoms of CAG are abdominal pain,pain relief during gastric emptying,aggravating symptoms of eating or satiety,heartburn,acid reflux,etc.When the disease is serious,patients may have anemia,emaciation and other manifestations.In recent years,the incidence of CAG has increased year by year,and the average age of patients tends to be younger.And CAG is characterized by chronic,long-term,repeated,so the most important thing is to find a way to cure CAG thoroughly.At present,gastroscopy and pathology are usually used to diagnose CAG In recent years,a new non-invasive method,serological screening,has been widely used in the early screening of gastric cancer.At present,the pathogenesis of CAG is still unclear,but it is generally believed that CAG is caused by multiple endogenous and exogenous factors(such as emotional stress,physical and chemical factors and microbial infection)caused by repeated gastric mucosal damage.In-depth studies on the pathogenesis of CAG have focused on the treatment of CAG However,there is no effective treatment for CAG except drug therapy and elimination of some causes(helicobacter pylori eradication triple drug,gastric acid inhibitor,and discontinuation of NSAIDs).Therefore,it is very important to develop more effective CAG treatment methods.Traditional Chinese medicine(TCM)is the "treasure" of China.In the course of its development for thousands of years,its treatment of "stomach diseases",especially chronic atrophic gastritis,has its own unique features.By summing up Dr.Sun Zhiguang’s rich experience in treating CAG in clinical practice for many years,the author created his own method of "invigorating qi,clearing heat and activating blood circulation"to treat CAG To study the clinical therapeutic effect of this method on CAG patients,and to carry out the experimental study on the efficacy of NGR1,an extract of Panax Notoginseng,on chronic atrophic gastritis,in order to give full play to the characteristics of traditional Chinese medicine"treating both the symptoms and the symptoms",so as to achieve better results in the treatment of chronic atrophic gastritis.Objective(1)The serum levels of PG I,PG II and G-17 in CAG patients at different stages and the effects of the method of invigorating qi,clearing heat and activating blood circulation on CAG patients were studied.(2)To study the effect of NGR1 on CAG rat model and discuss its potential mechanism.Methods(1)From January 2015 to December 2017,300 gastroscopic patients were selected as the study subjects.Among these 300 patients,196 were CAG patients.196 patients with CAG were divided into two groups according to random number table method,namely treatment group and control group.The number of the two groups was the same.There were 104 non-CAG patients.Statistical tests showed that there was no significant difference in basic data between CAG group and non-CAG group,treatment group and control group(p>0.05).The levels of PGI,PGR(PGI/PGII)and G-17 were measured by immunoradiometric method.The results of CAG group and non-CAG group were compared and analyzed.In the treatment,the control group used the previous treatment program,while the treatment group used the Yiqi Qingre Huoxue program,and observed the treatment of two groups of patients.(2)The experimental study used CAG rats as the target treatment.After NGR1 treatment,the results were obtained by histopathological observation,enzyme-linked immunosorbent assay(ELISA),Western blot analysis,quantitative RT-PCR,etc.After processing,draw a conclusion.Results(1)Compared with non-CAG group,the serum levels of PGI,PGR and G-17 in CAG group were lower(p<0.05).The overall therapeutic effect of the treatment group is better than that of the control group.(χ2=13.833,p=0.0002).(2)NGR1 treatment has a significant effect on the protection of rat CAG;NGR1 significantly increased the levels of GAS and SS and decreased the level of MTL in serum of CAG rats;NGR1 significantly decreased the levels of IL-1β and IL-6 in rats;NGR1 decreased Serum PGE2,NOS and ET levels in CAG rats;NGR1 significantly inhibited the decrease of sIgA and GSH levels in rat serum;increased expression of Bcl-2 and decreased Bax expression in gastric tissue of CAG rats were dose-dependently treated by NGR1 eliminate.Conclusion(1)The method of detecting PG and G-17 can be used for the screening of CAG patients.At the same time,the clinical effect of the method of invigorating qi,cleariing heat and activating blood circulation is better for CAG patients.(2)NGR1 attenuates rat CAG by regulating the secretion of gastrointestinal hormones,inhibiting the production of inflammatory factors,reducing the expression of gastric mucosal destruction factors,increasing the expression of gastric mucosal protective factors,and regulating the expression of Bcl-2 and Bax. |
