| ObjectsTo initially study the rules of TCM syndrome elements and the features of symptoms in patients with post-stroke cognitive impairment,investigate characteristics of TCM pathogenesis of post-stroke cognitive impairment,so as to provide a foundation for further study on dialectical normalization of TCM on post-stroke cognitive impairment.Methods1.The research group developed the questionaire.Researchers were trained in the same standard to collect information.Cases of post-stroke cognitive impairment were selected from multi-centers,including hospitals in Beijing,Shandong,Hebei,Changchun,Xiamen and so on.2614 cases were finally included in the study.Further the cases were divided into a group without cognitive impairment(Group 01),a group with wild cognitive impairment(Group 02)and a group with wild dementia(Group 03)according to the level of cognitive dysfunction.2.A data management plantform was established based on Browser/Server architecture.The data management plantform has the functions of data input,data export,data check and data management.Statistics were processed by SPSS17.0.Introduce the diagnosis scale for TCM syndrome elements which were established in previous studies of our research team.Use structure program designing to determine the syndrome elements.3.Use SPSS 17.0 to process the data statistically.Single-syndrome element,two elements and the complication of elements were compared between the three different groups,in order to analyse the general rules of TCM syndrome elements.4.Use correspondence analysis to investigate the features of symptoms in patients with post-stroke cognitive impairment,future analysis the TCM pathogenesis.Results1.In the aspect of Single-syndrome element,the proportion of phlegm in group 01 was the highest(40.3%).Hower,qi deficiency share great majority in group 02 and 03,run to 45.3%and 52.8%separately.In the three different groups,the incidence rates for elements of qi deficiency,blood stasis,yang deficiency and yin deficiency increased between any two groups of them(P<0.0167).The incidence rates for elements of fire and phlegm increase between group 01 and 03(P<0.0167).2.In the aspect of the score of the syndrome elements,the values of qi deficiency and yang deficiency were increased between group 01 and 02(P<0.0167),and the same can be seen in syndrome element of blood stasis between group 02 and 03(P<0.0167).Though the fire and phlegm elements had no difference between group 01 and 02 or between group 02 and 03(P>0.0167),the statistics difference can be found between group 01 and 03(P<0.0167).3.In the aspect of complication of elements,two syndrome elements was the most common components in the three groups(24.4%,28.0%,21.7%,respectively).The average number of elements were increased(P<0.01),and the specific average number were 1.94±1.40,2.24±1.49 and 2.70±1.65.4.In the aspect of two syndrome elements,the main situation were phlegm+blood stasis(16.8%),qi deficiency+phlegm(16.7%)and qi deficiency+yin deficiency(16.4%)in group 01.The main situation were qi deficiency+yin deficiency(23.5%),qi deficiency+yang deficiency(21.4%),qi deficiency+phlegm(20.8%)and qi deficiency+blood stasis(19.8%).The main situation were qi deficiency+yin deficiency(29.5%),qi deficiency+yang deficiency(29.3%),qi deficiency+phlegm(28.9%)and yin deficiency+yang deficiency(28.2%).In addition to fire+ phlegm and phlegm+yin deficiency,the proportion of any situation were increased between group 01 and 02,and the same can be seen between group 02 and 03(P<0.01).5.According to the results of correspondence analysis,X2=767.968,P<0.05,there has a obvious relationship between the six syndrome elements.The relationship between qi deficiency and yin deficiency was the closest,the correlation coefficient is 0.13155.And the correlation coefficient between qi deficiency and yang deficiency is 0.27127,the correlation coefficient between yang deficiency and yin deficiency is 0.28093.6.According to the results of Spearman correlation analysis of syndrome elements and sub-items,qi deficiency had a positive correlation with orientation,attention and calculation,naming,abstraction,delayed memory.Fire had a positive correlation with orientation.Phlegm had a positive correlation with naming,attention and calculation.Yang deficiency had a positive correlation with immediate memory,delayed memory.Yin deficiency had a positive correlation with attention and calculation.7.According to the results of Spearman correlation analysis of syndrome elements and the course of stroke,in group 01,the course had a positive correlation with qi deficiency and yang deficiency,and the correlation coefficient is 0.06 and 0.103,while it had a negative correlation with phlegm,and the correlation coefficient is-0.062.In group 02,the course had a positive correlation with qi deficiency,yang deficiency and yin deficiency,and the correlation coefficient is 0.14,0.175 and 0.12 respectively.In group 03,the course had a positive correlation with qi deficiency and yang deficiency,and the correlation coefficient is 0.163 and 0.118.Conclusions1.Qi deficiency,fire,phlegm,blood stasis,yang deficiency and yin deficiency lying in the whole process of post-stroke cognitive impairment.2.In the stage of onset,the existence of qi deficiency,blood stasis,yang deficiency and yin deficiency may bring a decrease in the level of cognitive function.And the risk of cognitive impairment may increase with the aggravation of qi deficiency or blood stasis.In this process,the role of qi deficiency is the most significant,and it often appears together with yin deficiency.In the stage of disease progression,the six syndrome elements can affect the cognitive function respectively.The risk of cognitive impairment may increase with the aggravation of blood stasis.However,the syndrome elements of fire and phlegm may related to the fluctuation of disease or the rapid decline of cognitive function.3.The combination of syndrome elements is becoming more and more complex with disease progression.The most common syndrome type is deficiency of both qi and yin syndrome.Followed by deficiency of both qi and yang syndrome,and deficiency of both qi and yang syndrome.4.Liver,spleen and kidney are the main disease location,among which kidney is the most important.5.Syndrome elements affect the level of cognitive function by acting upon different cognitive domains.6.The deficiency in root plays a dominant role through the whole disease.The deficiency of kidney qi,essence and yang usually exist early in the disease.As the illness progresses,deficiency of yin both of kidney and liver or deficiency of yang both of kidney and spleen appear,and finally become both yin-yang deficiency.In the aspect of complication,phlegm and blood stasis lying in the whole process of disease.Therefore,great importance should be attached to replenishing kidney essence and yang.And remove blood stasis and dissipating phlegm in the meanmine time. |
