| Wang Chunsheng,director of the physician,is the fourth group of Beijing municipal famous old Chinese medicine experts,the fifth group of national named Chinese medicine experts,learned from the country-named old Chinese medicine expert Xu Pengling.He advocates spleen-stomach doctrine,dedicated to research gastrointestinal disease,peruse TCM classics,deeply studies the expositions on the spleen and stomach to learn the essence,and formed his own academic thought of harmonising the spleen and stomach.He specializes in the treatment of digestive system disorders,and harmonises spleen and stomach to treat kinds of diseases of internal medicine.This paper is divided into four parts:The first part introduces the origins of professor Wangs academic thought,the second part summaries professor Wang’s academic thought.and clinical experiences,the third part retrospectively studies teacher Wang Chunsheng’s experiences of treating chronic gastritis using TCM inheritance auxiliary platform with data mining method,the fourth part of a case-control method to prove the clinical effect of professor Wang’s Kai Pi Huo Xue Decoction to chronic atrophic gastritis of the syndrome of dampness-heat of spleen and stomach.The first part introduces the origins of Professor Wang’s academic thought.Wang was born in a TCM family.Family influence and formal learning combination laid a solid foundation.He studied TCM classics diligently and the classics were the important bases of his academic thought.He learned from expert Xu Pengling,and respect the patients that the patients were the best teachers.He can advance with the times and develops his own ideas of some diseases.The second section summarizes Wang’s academic thought and clinical experiences of treating the disorders of spleen-stomach system and other internal diseases.Wang believes TCM is a gentle medicine because it is influenced by Chinese ancient naive materialism theory and natural science subject.Chinese medicine use herbs to correct the disorder of the human body,so as to achieve a balance status of yin and yang.Wang believes various diseases all due to the "imbalance",so can be cured with harmonization method.The harmonization methods include harmonizing spleen-stomach and balancing yin-yang.Then here summarizes Wang’s academic experiences of the treatment of digestive system diseases:chronic gastritis,peptic ulcer,gastroesophageal reflux,functional dyspepsia,constipation,as well as hypertension,menopausal syndrome,diabetes and uterine fibroids.Part Ⅲ:Objective:To summarize Professor Wang’s laws of medicine application of treating chronic gastritis.Methods:202 cases of chronic gastritis from Wang’s specialist outpatient from October 2013 to October 2014 are retrospectively arranged,then the four diagnostic information of looking,listening,questioning and feeling the pulse logged in the database,using TCM inheritance auxiliary platform V2.5 to analyze.Results:there are 129femalepatients,accounting for 64%,and73 males accounted for 36%in the 202 cases,a total of 108herbal medicine,in which the top five were Fuling198 times,chao Baizhu 185times,Baikou174times,Qiao sharen 147 times and Muxiang 135 times.Besides coating and pulse,the front seven symptoms are gastrectasia,stomachache,belching,acid reflux,insomnia,dry mouth,loss of appetite.Syndromes occur mainly the syndrome of weakness of spleen and stomach 116 times,the syndrome of liver-stomach disharmony 71 times and the syndrome of dampness-heat of spleen and stomach 34 times.Therapeutic treatment methods are strengthening the spleen and stomach,dispersing stagnated liver qi for regulating stomach,mildly regulating cold and heat.Prescription rules analysis based on the law of apriori,choosing support 40%and confidence level>95%of the drug combination,there are nine herbs:Chao baizhu,Fuling,Baikou,Qiaosharen,Houpo,Fabanxia,Huanglian,Shengyimi,Muxiang,raising supportup to 50%,there are seven herbs:Chao baizhu,Fuling,Baikou,Qiaosharen,Houpo,Muxiang,The potential drug combination can be tapped by "Entropy Clustering" and "improvement of mutual information".New prescriptions can be drew based on unsupervised hierarchical clustering entropy.The herb-components of the syndrome of weakness of spleen and stomach are Dangshen,Fuling,Chao baizhu,Baikou,Banxia,Houpo,Qiaosharen,Muxiang.The herb-components of the syndrome of liver-stomach disharmony are Chaihu,Zhiqiao,Chao baizhu,Fuling,Qiaosharen,Muxiang,Baikou,Houpo.The herb-components of the syndrome of dampness-heat of spleen and stomach are Banxia,Huanglian,Ganjiang,Dangshen,Chao baizhu,Fuling,Baikou,Qiaosharen,Houpo,Muxiang.Conclusion:The basic pathogeny of chronic gastritis is improper diet and poor emotion,the interpretation of the cause is weakness,the main pathological factor is wetness,then indigestion,blood stasis and toxin.Spleen,stomach,liver is the main location,with the distention and fullness,gastric abscess as the main symptom.The main syndromes are the syndrome of weakness of spleen and stomach,the syndrome of liver-stomach disharmony and the syndrome of dampness-heat of spleen and stomach,with the therapeutic treatment methods of strengthening the spleen and stomach,dispersing stagnated liver qi for regulating stomach,mildly regulating cold and heat.The prescription of the syndrome of weakness of spleen and stomach is XiangshaLiujunzi decoction,the prescription of the syndrome of liver-stomach disharmony is XiangshaLiujunzi decoction without Dangshen,but adding Chaihu,Zhiqiao,the prescription of the syndrome of dampness-heat of spleen and stomach is Banxiaxiexin decoction and Xiangsha Liuj-unzi decoction with a little adding and subtracting.Part Ⅳ:First is the chronic atrophic gastritis summary of Chinese and Western medicine,then the experimental part.Objective:case-control study to verify the exact effect of professor Wang’s Kai Pi Huo Xue Decoction to chronic atrophic gastritis(CAG).Methods:This test had a total of 60 patients that were both from Wang’s specialist outpatient,randomly divided into treatment group and control group with 30 cases in each.The treatment group taken Kaipihuoxue decoction twice a day half an hour after breakfast and supper,and once 200ml.The prescription of Kaipihuoxue decoction is Banxiaxiexin decoction adding Danshen,and the ingredients are Banxia 9g,Huanglian 6g,Huangqin 6g,Ganjiang 6g,Dangshen 10g,Dazao 6g,Zhicao 6g,Danshen 15g.The control group taken Moluo Dan three times a day and 8 pills once.The patients of the two groups had no significant differences in gender,age,symptoms,pathological changes in the gastroscope(P>0.05).The period of treatment was 6 months,and the patients were forbidden to take other medications for chronic gastritis during the period.The symptoms were record every 3months.Gastroscopy was reviewedafter6 months of treatment.Results:l.The symptom score of the patients of the two groups are both significantly decreased after three months treatment and six months treatament(P<0.05),and the curative effect of the treatment group is better than which of the control group(P<0.05);2.The pathological score of the patients of the two groups are both significantly decreased after six months treatment(P<0.05),and the curative effect of the treatment group is better than which of the control group(P<0.05);Although there is no statistical significance in the atrophy,intestinal metaplasia,chronic inflammation,activity individual score before and after the treatment,there is great clinical significance,and the curative effect of Kaipihuoxue decoction is better than which of Moluodan,especially for intestinal metaplasia.3.From the curative effect of TCM syndrome,for the treatment group there areO cases of well-healed,17 cases of markedly effective,13 cases of effective,0 cases of no effect,the total efficiency of 100%,with 56.67%markedly efficiency;for the control group there are 0 cases of well-healed,5 cases of markedly effective,25 cases of effective,0 cases of no effect,the total efficiency of 100%;withl6.67%markedly efficiency,There are significant differences in markedly effective rate between the treatment group and the control group,and the curative effect of the treatment group is better than which of the control group(P<0.05).4.From the gastroscope,the curative effect of the treatment group is better than which of the control group in the aspects of mucosal white phase or piebald,folds flat,rough mucosa,intestinal metaplasia nodules and mucosal bleeding(P<0.05).The curative effect of the treatment group is better than which of the control group in the aspects of erythema,erosion,vascular penetration,bile regurgitation,but it has no statistically significance(P>0.05).Conclusion:Kaipihuoxue decoction can cure chronic atrophic gastritis with the syndrome of dampness-heat of spleen and stomach.It can not only improve symptoms,but also can reduce and reverse the pathological change of pathological atrophy,intestinal metaplasia. |
