Diabetes mellitus, is a major epidemic of this century, which has increased in incidence by50%over the past10years. The World Health Organization states that347million people worldwide were suffering from diabetes in2008, which equates to9.5%of the adult population. The incidence of diabetes is rapidly increasing with estimations suggesting that this number will almost double by2030. Diabetes mellitus occurs throughout the world but is more common in developed countries. The greatest increase in prevalence in the near future, however, is expected to occur in Asia, the Middle East (4), and Africa, where it is likely that there will be an50%increase in diabetes in these parts of the world by2030. Chronic vascular complications accounts for main morbidity and mortality of diabetic patients and the insidious progress of type2diabetes mellitus add to the risk for development of all kinds of complications. The reasons for hospitalization of diabetic patients vary due to the course of the disease. A national retrospective analysis on chronic diabetic complications and their risk factors has been published in2003, and the results indicate that the prevalence of diabetic chronic complication is rather high. We therefore attempted to evaluate the reasons for hospitalization and the frequency of different chronic complications and the related risk factors in type2diabetic patients in Zhejiang Province thus we aimed to give a rational insight into the regional situation as well as provide an impetus for wider introduction of preventive approaches. Part one:Reasons of Hospitalization for Type2Diabetes MellitusOBJECTIVE:Diabetes Mellitus is the most common chronic disease in China and its chronic complications accounts for main morbidity and mortality of the diabetic patients, greatly increasing economic burden. We there for attempted to investigate the reasons for hospitalization of Diabetes Mellitus, Thus we aimed to give a rational insight of the regional situation as well as to provide an impetus for wider introduction of preventive approaches.METHODS:We retrospectively analyzed the clinical and laboratory data of597type diabetic inpatiants in the Affiliated Sir Run Run Hospital of Zhejiang University during Jan1,2014to Dec31,2014. The reasons for hospitalization were confirmed mainly according to the chief complaint of the patients and the diagnosis on discharge.T-test was applied to determine the difference between two groups. All data was processed by SPSS20.0. P<0.05was considered statistically significant.RESULTS:The two predominant reasons for hospitalization of diabetes were hyperglycemia and neuropathy,61.1%and12.7%respectively. Other main reasons for hospitalization were ophthalmopathy11.6%, nephropathy8.4%, acute complications6.7%, infection2.7%and diabetic foot1.5%.CONCLUSIONS:The dominant reason for hospitalization of diabetes was hyperglycemia and chronic diabetic complications. Data were collected according to the unified strategy in the program of the ten-year-retrospective analysis of diabetic chronic complications in Chinese diabetic inpatients launched by Chinese Diabetes Society, Chinese Medical Association in2001.We analyzed the occurrence of cerebrovascular diseases, cardiovascular diseases, peripheral vascular diseases, retinopathy, nephropathy, neuropathy and their risk factors. Multivariate logistic regression model was used to identify independent risk factor for chronic complications. All data was processed by SPSS20.0. P<0.05was considered statistically significant.RESULTS:The prevalence rates of chronic diabetic complications were:8.88%for cardiovascular disease,13.32%for cerebrovascular disease,23.12%for nephropathy,28.31%for retinopathy,41.04%for peripheral neuropathy. The risk factors for chronic diabetic complications were:age, sex, duration of diabetes, systolic blood pressure, dyslipidemia, HbAlc and homocysteine. CONCLUSIONS:The prevalence of chronic diabetic complications in Zhejiang province was high. Current therapy should cover controllable risk factors including hypertension, hyperglycemia and dyslipidemia.
