| Objectives:Traditional Chinese Medicine(TCM) occupying important position in china is a effective way with little side-effect to treat bronchial asthma in Remission Period gauided by the idea of preventive treatment of disease. Wenyanghuatanfang and Qingyanghuatanfang which are two experience prescribes formulated by Traditional Chinese Medicine great master Zhou Zhongying based on the theary of the wind phlegm and stubborn root and the treating the secondary aspect of disease as being ill and treating the root as being steady. This theory is the fruit of study about the two prescribes to carry forward the experience and method of professor Zhou Zhongying who has put forwad the therapy method of boosting lung and kidney and dispelling wind and phlegm. On the basis of the research, we discussed the theory, method, formula and herbs and probe the mechanism of action,so as to study the experience of professor Zhou Zhongying.Methods:In the part of clinical observation, adopting the method of cohout study,211 patients of Bronchial Asthma in Remission Period accompanied with 9 patients dropping or rejected. In actually, the data of 202 cases was analysed. There were 73 cases in cold syndrome type,48 cases in heat syndrome type,121 cases were all brought into the group of famous TCM. There were 44 cases in cold syndrome type,37 cases in heat syndrome type,81 cases were all brought into the group of routine TCM.4 groups of patients took respectively Wenyanghuatanfang, Qingyanghuatanfang, Gubenkechuan decoction and Maiweidihuang decoction for 24 weeks. To observe the changes of symptom, sign, ECP, IL-2, IL-4, IL-13, IFN-γetc.In the part of experimental research, one hundred and thirty-five male Wistar rats were divided randomly into nine groups:Normal control group, Asthma model control group, Dexamethasone group, Qingyanghuatanfang prescription low dose group, Qingyanghuatanfang prescription middle dose group, Qingyanghuatanfang prescription high dose group, Wenyanghuatanfang prescription low dose group, Wenyanghuatanfang prescription middle dose group, Wenyanghuatanfang pill high dose group, each group had fifteen rats. By the way of being sensitized and long term inspired with ovalbumin, we copy chronic asthma rats model. At the same time, each group was given relevant medicines and observed about normal symptoms. At the end of the experiment, all the rats were killed. We observed the changes of lung histomorphology of rats of each group, and detected TNF-a in the serum by enzyme-linked immunosorbent assay and expression of NF-κB and MIF in the lung by immunohistochemistry. The data of NF-κB and MIF is quantitative image analysis. Experimental data were analyzed by spssl5.0 statistical software.Results:In the part of clinical observation:1.In the group of cold syndrome type, the control rate was 20.0%, the clinical control obviously rate was 74.0%, the total effective rate was 98.6% in the group of famous TCM, with 13%,43.2%,84.1% in the group of ordinary TCM. there was significant difference between the two groups (P<0.01).2. In the group of heat syndrome type, the control rate was 22%, the clinical control obviously rate was 68.8%, the total effective rate was 91.7% in the group of famous TCM, with 8.0%,35.1%,78.4% in the group of ordinary TCM. there was significant difference between the two groups (P<0.01).3. On the whole, the group of famous TCM got more clinical effective at the aspect as the forward effective, the quality of life, ECP, IL-2, IL-13, IFN-γetc than the group of ordinary TCM.In the part of experimental research, the chronic asthmatic rat models were successfully made. The normal symptoms and the changes of lung histomorphology of Every treatment group is better than Asthma model control group. Compared to Normal control group, Asthma model control group was a large number of inflammatory cell infiltration, airway epithelial injury and airway wall thickening. Pathological changes in the treatment group compared with the asthma model group, Qingyanghuatanfang prescription low group, Wenyanghuatanfang prescription middle group and Wenyanghuatanfang prescription high group are most clearly reduce. About the expression of NF-κB in lung tissue, Asthma model control group was significantly higher than Normal control group; each treatment group was significantly lower than Asthma model control group (P<0.05), but significantly higher than Normal control group (P<0.01); compared with Dexamethasone group, Wenyanghuatanfang prescription middle and high group and Qingyanghuatanfang prescription low group were no significantly different (P>0.05), other traditional Chinese medicine group were significantly higher than Dexamethasone group (P<0.05); the expression of NF-κB of Wenyanghuatanfang prescription high grpup is lowest which is significantly different with Qingyanghuatanfang prescription high group (P<0.05). About the expression of MIF in lung tissue, Asthma model control group was significantly higher than Normal control group; each treatment group was significantly lower than Asthma model control group (P<0.05), but significantly higher than Normal control group (P<0.01); compared with Dexamethasone group, every group of Huatanfang prescription is no significantly different (P>0.05); the expression of MIF of Wenyanghuatanfang prescription high grpup is lowest which is significantly different with Qingyanghuatanfang prescription high group (P<0.05). The serum TNF-αof Asthma model group and treatment groups were significantly higher than Normal control group(P<0.05); every treatment group was significantly lower than Asthma control group (P<0.05); traditional chinese medicine group reduce the level of serum TNF-αwhich is no significantly different with Dexamethasone group (P>0.05), Qingyanghuatanfang prescription high group and Wenyanghuaranfang prescription high group are the most obvious.Conclusions:Wenyanghuatanfang and Qingyanghuatanfang can obviously improve patients' symptom and sign like gasp, cough, choking sensation and wheezing sound, increase the score of ACT. Meanwhile the two prescribe can improve the forward effective and the quality of life with obvious superiority compared with ordinary TCM. The result of the study points out the good safety with light untoward effect.In the part of experimental research:1. The experimental results suggest that Huatanfang prescription can effectively alleviate the clinical symptoms and lung histomorphology changes of chronic asthmatic rats, and its mechanism is to:reduce the NF-κb and MIF expression and serum TNF-αcontent.2. Different therapies about Huatanfang prescription both can reduce the expression of NF-κB and NIF and serum TNF-αlevel, but there are differences in terms of dose,which may be because the experimental model does not have the "syndrome" feature on. |
