Study On Acupuncture Therapy Of Dredging Governor Vessel For Regulating Mentality In Post Ischemic Stroke Hemiplegic Patients

Objective:To study the efficacy and mechanism of acupuncture therapy of dredging governor vessel for regulating mentality in post ischemic stroke hemiplegic patients, and provide a scientific theory and practical evidence for its further spread and application.Methods:Prospective, randomized, controlled trial with a large sample.123 participants were divided into the following three groups:(1) Therapy group 41 cases were treated with acupuncture therapy of dredging governor vessel for regulating mental ity addition to the basic treatments in western medicine. Points GV26, GV24, GV23, GV20, GV16, GV14and GV3 were selected,3-4 points once alternatively, and auxiliary points selected on syndrome differentiation, once daily with 30 min retention including needle manipulation per 15 min, all for 14 times; (2) Routing group 41 cases were given routine acupuncture treatment plus the basic treatments. Points GV26, PC6, SP6, HT1, LI11, GB30 and GB34, 3-4 points once alternatively, and auxiliary points selected on syndrome differentiation, once daily with 30 min retention including needle manipulation per 15 min, all for 14 times; (3)Basic group 41 cases just only basic treatments,30 min once daily, for 14 times. Basic treatments included with the drug therapy and the rehabilitation training program. The drug therapy, referring to China Guidelines on Prevention and Management of Cerebral Vascular Diseases, published by Ministry of Health Disease Control Division of Neurology Institute of the Chinese Medical Association 2005. And the rehabilitation training program was mainly based on the 3rd edition of the Public Health Department planned textbook Medical Rehabilitation published by People's Health Publishing House. All the patients were measured with National Institute of Hospital Stroke Scale (NIHSS) for neurologic deficit evaluation, Barthel Index (BI) for daily living activities assessment, Simplified Fugl-Meyer Assessment (FMA) for motor function appraisement and Modified Ashworth Scale (MAS) for muscle spasticity rating before and after treatment.Results:It was comparable among the three groups in gender, age, course of disease and evaluation scores of NIHSS, BI, FMA and MAS before the treatment (P>0.05).1 Changes in NIHSSAfter the treatment, NIHSS evaluations of the therapy group and routine group were significantly decreased than before (P<0.05); while that of the basic group was not (P>0.05). There was obvious difference of NIHSS evaluations before and after the treatment among three groups (P<0.05)。And there were significant differences between every two groups (P<0.05) Among three groups, the therapy group was the best to relieve patients' neurologic deficit, the routine was the better and the basic the worst.2 Changes in BIAfter the treatment, BI measurements of the therapy group and routine group were significantly increased than before (P<0.05); while that of the basic group was not (P>0.05).There was obvious difference of BI measurements before and after treatment among three groups (P<0.05). Between every two groups, the therapy group was significantly different from the other (P<0.05), and there was no significant difference between the routine group and basic group (P >0.05). Among three groups, the therapy group was the best to promote patients ' daily living activities, the routine was the better and the basic the worst.3 Changes in Simplified FMAAfter the treatment, Simplified FMA assessments of the therapy group and routine group were significantly increased than before (P<0.05); while that of the basic group was not (P>0.05).There was obvious difference of FMA assessment before and after the treatment among three groups (x2=20.387 v=2 P=0.000). Between every two groups, the basic group was significantly different from the therapy group and the routine group P<0.05), and there was no significant difference between therapy group and the routine group (P>0.05) Among three groups, the therapy group was the best to enhance patients' motor function, the routine was the better and the basic the worst.4 Changes in MASAfter the treatment, MAS qualifications of the therapy group was significantly decreased than before (P<0.05); While that of the routine group and basic one were not. (P> 0.05).There was obvious difference of MAS qualification before and after the treatment among three groups (P<0.05). Between every two groups, the therapy group was significantly different from the other two (P<0.05). Among three groups, the therapy group was the best to alleviate patients' muscle spasticity, the routine was the better and the basic the worst.Conclusions:Acupuncture therapy of dredging governor vessel for regulating mentality, which possesses advantages in aspects of relieving neurologic deficit, promoting daily living activities, enhancing motor function and alleviating muscle spasticity can improve living quality and shorten recovery duration of post ischemic stroke hemiplegic patients. It is helpful for them to return back to family and community early, and is worth of further popularizing and applying.
