ObjectiveTo further discuss the mechanism of 3-Leg point in Jin'3-needle technique on regulating abdominal postoperative gastrointestinal motility, this research stdied the effects of acupuncture on regulative mechanism of thick myofilament in smooth muscle contraction and gastrointestinal pacing cell ICC in abdominal postoperative rat. This could offer a scientific theoretical basis to further clinical application of acupuncture treatment on abdominal postoperative gastrointestinal dysmotility.MethodLiterature researchReviewed literatures on regulative mechanism of thick myofilament in smooth muscle contraction, researches of ICC, clinical researches and experimental researches of postoperative gastrointestinal dysmotility in the past 20 years, summarized the understanding of domestic and foreign scholars on postoperative gastrointestinal dysmotility, the study methods of gastrointestinal movement, the machanism study and the treatment progress in this field.Experimental research40 rats were devieded randomly into acupuncture group, model group, sham-operation group and blank group, each group 10 rat. Rats in acupuncture group and model group were given colon transection and anastomosis. Rats in sham-operation group were given open abdominal operation. Both acupuncture group and sham-operation group were given acupuncture treatment when came round after operation. The 3-Leg point in Jin' 3-needle technique (Zusanli, Sanyinjiao, Taichong) was used. Inserted the needle and do manipulation like lifting and thrusting, twirling and rotating 2-3 times every 5 minutes. The treatment lasted 15 minutes and was given once a day for 3 days. The model group and blank group were trapped into the same self-made fixator for 15min each day.Observed and recorded the time of first defecation after operation, everyday's weight of excrement in the first 3 days after operation. Then rats were killed and intestinal propulsive rate was measured. Tissue in pacemaking eara of gas, which was in the middle part of gas 1/3 from cardia and tissue of colon which was 2cm under caecum (anastomosis part) were taken and divided into 3 parts:one was used to make pathology section to observe tissue contents of myosin light chain kinase, myosin and actin; one was used to make electron microscope specimen to observe ultrastructure of ICC; the last one was used to detect gene expression of CALM1 mRNA by RT-PCR.Result1. Defecation:The first defecation time after operation in acupuncture group was significantly shorter than that in blank group(P<0.05). The decrease of excrement weight was significant different in acupuncture group, model group and sham-operation group from that in blank group(P<0.01). Compared the acupuncture group with model group, there was no significant difference on the first day after operation, but from the second day on the difference became significant P<0.05).2. Intestinal propulsive rate:After operation the intestinal propulsive rate of model group and blank group was significant different (P<0.05). Also there is significant difference between acupuncture group and model group(P<0.05).3. The gene expression of CAM1 mRNA change:There was no significant difference between acupuncture group, model group and blank group(P>0.05).4. Gastrointestinal tissue contents of myosin light chain kinase:The content of MLCK in colon smooth muscle in blank group is highest, that in model group decreased significantly(compared to blank group P<0.05). While that in acupuncture group increased compared to model group(P<0.05). The content of MLCK in colon mucosa epithele was just opposite. The content of MLCK in stomach smooth muscle and mucosa epithele had nearly no changed (P>0.05) among the three groups.5.Gastrointestinal tissue contents of myosin:The contents of myosin in colon in blank group is highest, that in model group decreased significantly(compared to blank group P<0.05). While that in acupuncture group increased compared to model group(P<0.05). The content of myosin in stomach smooth muscle had nearly no changed(P>0.05) among the three groups.6. Gastrointestinal tissue contents of smooth muscle actin:The contents of smooth muscle actin in model group decreased significantly(compared to blank group P<0.05). While that in acupuncture group increased compared to model group (P<0.05). The content of myosin in stomach smooth muscle had nearly no changed(P>0.05) among the three groups.7. Gastrointestinal tissue contents of C-kit:The contents of C-kit in model group decreased significantly(compared to blank group P<0.05). While that in acupuncture group increased compared to model group(P<0.05). The content of C-kit in stomach smooth muscle had nearly no changed (P>0.05) among the three groups.Conclusion1. Acupuncture can shorten the time of first defecation after abdominal operation, increase the quantiity of defecation and improve the intestinal motility.2. The regulative effects of acupuncture on mechanism of thick myofilament in smooth muscle contraction is relevant to the effects of chaning gastrointestinal tissue contents of myosin light chain kinase, myosin, smooth muscle actin and C-kit. And there is no relevant to the gene expression of CAM1 mRNA.3. Acupuncture can increase ICC' s number and better ICC s ultrastructure. This is perhaps one of the mechanism of the regulation of acupuncture on abdominal postoperative gastrointestine motility.
