Currently in our country, medical resources development level does not match with the level of social economic development, and the distribution and configuration of medical resources is also imbalance. The main problem is that medical resources in China are concentrated in large hospitals, which has resulted in the serious lack of development opportunities and technological capabilities for grassroots services. The telemedicine is an essential and effective method to break this current dilemma. In recent years, our country has made large investments on telemedicine and introduced relevant policies to provide support and encouragement. After a lot of policies and resources are carried out, however, most of telemedicine equipment is out of use and the telemedicine is proved generally ineffective. The tremendous tasks confronting us are as follows: how to make the maximum utilization of telemedicine equipment resource,how to integrate the resources of medical institutions, how to improve the diagnosis and treatment level of grassroots services and how to make digital major public hospitals become the central part of entire healthcare management process.Under the support of several funds,this thesis makes a research on the telemedicine reform in the Internet of Things( Io T) mode. By applying Io T techniques and telemedicine techniques, this thesis has explored the way of quickly copying and efficiently operating the medical resources and education resources of large public hospitals, studied and formulated a new operation mode of telemedicine by establishing the telemedicine platform based on networking technology, and developed new applications of telemedicine. During research and implementation process, this paper proposed the zero-marginal-cost model to replicate the resources of large public hospitals and pioneered the way to diffuse high quality resources of these hospitals by establishing several specialized medical centers. It also proposed the model of telemedicine working that medicine and education played the role together, and integrated consultation system with education system so that consultation could be directly translated into education activities and the new model of telemedicine work has been explored. The model of distance education that learners are thought as the driver of this activity is proposed in which primary hospitals address their needs of courses and then large public hospitals organize courses to carry out distance education. In addition, this paper made useful exploration in remote outpatient registration, remote specialist diagnosis, remote monitoring, and remote health education based on specialist and so on.In exploring a new telemedicine model, this paper analyzed the dilemma of domestic telemedicine and the difficulties of telemedicine in large public hospitals, formulated the telemedicine system framework of large public hospitals, and dissected the implement of this system with TOGAF method, to cope with needs of system of the future telemedicine. This also points the direction of future technology development and system integration.In system design and technical implementation, the communication mode of perception layer and network layer in Io T device access is put forward and defined, solving the problem that how the equipment, which is not connected to the network, to join the telemedicine system. This thesis proposed solutions that integrate soft/hard videos of telemedicine device to solve the problem that hard video equipment and soft equipment can’t connect. In the platform layer of Io T, this thesis focused on how to use Nop Commerce technology for the framework design of the specialized centers. To realize the telemedicine with image, A DICOM viewer is intergreted into the system. To realize the interconnection and interworking among the different hospitals, this paper proposed solutions based on Webservice interface. To achieve the translation between consultation and education, this paper proposed technical proposals based on steaming media video servers. At the same time, to solve the problem of forwarding and calling of Io T monitoring device outside the hospital, the real-time data forwarding mechanisms and arithmetic avoiding conflicts of data gateway is researched and implemented.In summary, this thesis studied the theory and application of Io T technology, established the telemedicine system based on Io T, made model validation in pilot hospitals and gained initial results.
