| Objectives:This research is to observe the effect of combined treatment of recurrence of depression of and to explore the related factors of various symptoms and depression in general have effect on the recurrence of depression. Methods:This research adopts the method of prospective cohort study, including 127 patients from January 2017 to January 2018.According to the actual clinical medication, these patients were divided into Chinese medicine group,Western medicine group and combination of traditional Chinese and Western medicine group.Using the Montgomery-Asberg Depression Rating Scale(MADRS),the Social Disability Screening Schedule(SDSS)and the TCM Syndrome Scale as evaluation tools. The patients were followed up every 3 months after entering the group, and the scales was reassessed until the end of the study.Chi-square test was used to compare the relapse rate of depression between different treatment groups.Logistic regression analysis was used to study the difference of symptoms between relapse and non recurrence patients,so as to identify related factors of depression recurrence. Results:At the end of the experiment,127 patients were followed up,of which 41 were Chinese medicine group,42 in integrated Chinese and Western medicine group and 44 in western medicine group. The total score of MADRS of the three groups was in the normal distribution before the treatment after the normal test. The general data were not statistically different by t test or chi square test including gender, age, educational level, psychiatric history, family history, and total pre treatment.During the follow-up period,20 of the 127 patients(15.7%)had a relapse after a clinical cure. There were 7 relapses(17.1%)in the Chinese medicine group,6(14.3%)in the TCM and western medicine group,(16%)in the western medicine group. After the chi square test, there was no significant difference in the recurrence rate between the groups(P>0.05).Logistic regression analysis was used to detect the follow-up data. The three data of suicide idea, treatment effect(reduction rate),occupation and work social function were different between recurrence group and non recurrence group(P<0.05).In general, there was no statistical difference between the groups of sex, age, education, personal history and family history of mental illness(P>0.05).Symptoms of sleep disorders, eating disorders in MADRS and the shortness of breath, flank fullness in TCM Syndrome Scale and other symptoms in recurrence group and non recurrence group had no statistical difference(P>0.05).Conclusion:Traditional Chinese medicine, western medicine and traditional Chinese and Western medicine intervention method,in the first 3 months,to the depression patients in Kidney deficiency and stagnation of liver have a similar level of recurrence. There is a certain correlation between suicide concept, clinical efficacy(reduction rate),social function deficit and recurrence of depression. The stronger the concept of suicide, the worse the clinical effect(the lower rate of reduction),the greater the social function damage, the easier the recurrence of depression is.Gender, age of onset, family history of mental disorders, sleep disorders and eating disorders had no significant correlation with depression recurrence in this study. The long-term effect of depression on relapse of depression needs further observation. |
