The Study On The Treatment Of Soft Tissue Injury With Xiaoyanzhitong Ointment Massage Therapy

Objective:Soft tissue injury is common in clinical practice with various treatment methods.Ointment massage therapy has been an effective treatment for more than 2000 years and has been widely used in clinical practice.However,there are some problems such as low quality of clinical research and insufficient key basic research.Previous studies have shown that ointment massage therapy is effective in treating soft tissue injury.The purpose of this study was to systematically review the historical development and modern research of ointment massage therapy,preliminarily verify the safety and effectiveness of Xiaoyanzhitong ointment massage therapy through cell experiments,animal experiments and clinical studies,and elaborate its mechanism from the aspects of inflammation,oxidative stress and angiogenesis.Methods:1.Use CiteSpace and Excel software to comb the origin,historical development,modern research and prospect of ointment massage therapy from the aspects of ointment and massage.2.90 patients with soft tissue injury were included and divided into treatment group(n=60)and control group(n=30).the treatment group was treated with Xiaoyanzhitong ointment,while the control group was treated with infrared radiation.The main outcome measures were pain score after treatment,while the secondary outcome measures included 5-point satisfaction scale and functional assessment.3.Forty-two SD rats were divided into control group(n=6),ointment massage therapy group(n=18),and model group(n=18).SOD and MDA levels in serum were detected by acute blunt trauma model of rat gastritis muscle.H&E staining and immunohistochemical staining(CD34)of gastritis muscle were observed.4.The levels of IL-1β,IL-6,SOD and MDA of C2C12 after drug intervention were detected in Xiaoyanzhitong Ointment group,diclofenac sodium group,model group and control group.Results:1.Ointment massage therapy knowledge map includes 133 institutions,635 authors,and 352 keywords,forming four close teams with Cheng Yingwu and Tao Jiming’s team from Shanghai University of Traditional Chinese Medicine,Wang Xinjun and Liu Junchang’s team from Xinjiang Medical University,Ji Yuan’s team from Shandong University of Traditional Chinese Medicine,and Li Huanan’s team from Tianjin University of Traditional Chinese Medicine as the core.The research was published in 112 journals,mainly focusing on clinical efficacy observation,most of which had no fund support.The indications included more than ten systemic diseases,covering hundreds of diseases,among which motor system diseases accounted for about half.There were 22 manipulations,most of which used combination Therapy.There are 19 kinds of TCM,most of which are TCM for promoting blood circulation and removing blood stasis.There are 15 kinds of substrates,most of which are Vaseline.The preparation process is varied,and most of them are mixed with oil matrix after the research of TCM.The research focuses on TCM dermatology theory,lumbago disease,curative effect satisfaction evaluation.2.Clinical curative effect observation:Xiaoyanzhitong ointment group performed better in mean pain score,5-point satisfaction scale and functional assessment.3.Animal experiment:In the Xiaoyanzhitong ointment massage group,there were more newly formed muscle cells stained by H&E,less inflammatory infiltration,more normal shape,higher SOD level,lower MDA level,and more obvious red fluorescence(CD34).4.Cell experiment:C2C12 cells cultured with 2%horse serum showed obvious differentiation on the 5th day and reached the peak on the 9th to 10th day.C2C12 cell inflammation modeling method was 10ng/ml lipopolysaccharide treatment for 24h;the levels of IL-1β,IL-6 and MDA,and SOD were significantly inhibited by Xiaoyanzhitong ointment 1.25 mg/ml and diclofenac sodium solution 6.25 ug/ml for 24h.Conclusions:1.Ointment massage therapy has a long history,and related studies have gradually increased in the past 30 years.The clinical efficacy is mainly observed,but the quality is not high.At present,no unified and recognized technique and medication system has been formed,and basic research on key parameters such as frequency,duration and force of technique is lacking.Ointment are mostly oily substrates with poor drug release and transdermal properties,and modern pharmaceutical technology is not fully utilized.2.Clinical curative effect observation suggest that Xiaoyanzhitong ointment is better than infrared radiation in the treatment of chronic soft tissue injury.Animal experiments suggest that the treatment of Xiaoyanzhitong ointment massage therapy can effectively reduce the inflammatory response and oxidative stress damage after acute blunt trauma of gastrocnemius muscle in rats,accelerate local angiogenesis,and then accelerate the repair of damaged tissue.The cell experiment indicated that Xiaoyanzhitong ointment could reduce the levels of IL-1β,IL-6 and MDA and increase the SOD content in the lipopolysaccharide-induced inflammation model of C21C12 cells.
