Objective 1.By comparing the vestibular function test results of patients with vestibular paroxysmia(VP)and patients with benign paroxysmal positional vertigo(BPPV),to explore the characteristics and diagnostic value of vestibular function tests in VP;2.By comparing the neurovascular compression side suggested by MRI head and the side with impaired vestibular function suggested by vestibular function examination,we explored the consistency of the judgment of the vestibular function test on the side with impaired VP.Methods From August1,2016 to November 30,2020,60 cases of confirmed VP patients and BPPV patients were collected in the vertigo center,outpatient and inpatient departments of the Department of Neurology of Baotou Central Hospital.Confirmed VP group: meets the 2016 VP diagnostic criteria;BPPV group: meets the 2017 Bárá(?)y Society’s Vestibular Disease Classification Committee consensus to establish the posterior semicircular canal and horizontal semicircular canal BPPV diagnostic criteria.All subjects have completed detailed medical history collection and vestibular function examination.Inspection contents include: Video nystagmograph(VNG)inspection(saccade test,steady tracking test,optokinetic nystagmus test,gaze test,spontaneous nystagmus test,displacement test),temperature change test,video head pulse inspection(v HIT)and head MRI examination,the above data is statistically analyzed.Results 1.Compared with the BPPV group,the saccade test and displacement test in the vestibular function test of the VP group showed statistically significant differences(P<0.05).2.Compared with the BPPV group,in the vestibular function test of the VP group,there was no significant difference in optokinetic nystagmus,gaze test,spontaneous nystagmus,and steady follow-up test(P>0.05).3.7 cases(18.9%)of VP patients had no obvious abnormalities in the vestibular nerve function examination,22 cases(59.5%)of VP patients showed neurovascular compression on the same side as the vestibular nerve function decline or loss,and 8 cases(21.6%)VP patients The side of neurovascular compression is inconsistent with the side of vestibular nerve function decline or loss.Conclusion 1.The vestibular function test is not specific for the diagnosis of VP.2.The vestibular function examination of patients with VP indicated that the central and peripheral nerves were damaged.3.Combining vestibular function examination and cranial magnetic resonance imaging is helpful to judge the damaged side of VP.
