Correlation Between Symmetrical-dynamic Arteriosclerosis Index And Severity Of Coronary Artery Disease In Patients With Premature Coronary Heart Disease

Objective:Recent years,the onset age of coronary heart disease is getting younger,according to epidemiological investigation.As an index reflecting the severity of atherosclerosis,symmetrical-dynamic arteriosclerosis index has been confirmed to be related to coronary artery disease in patients with coronary heart disease.However,coronary artery disease in patients with premature coronary heart disease has its unique features.This study aims to explore the relationship between symmetrical-dynamic arteriosclerosis index and early-onset coronary heart disease Objective to investigate the correlation between the severity of coronary disease,so as to help the risk assessment in the the early stage and intervention of premature coronary heart disease,thus reduce the occurrence of fatal and malignancy cardiovascular events.Methods:From January 2019 to September 2020,147 hospitalized patients in the Department of Cardiology,the second affiliated hospital of Shanxi Medical University.147 patients met the inclusion criteria in this research had complete clinical data.General clinical data were collected and recorded ambulatory blood pressure monitoring and coronary angiography were performed,symmetrical-dynamic arteriosclerosis index was calculated by regression model.We use Gensini score system to reflect the severity of atherosclerosis in coronary artery.By the results of coronary angiography,we divided them into coronary disease group(n = 84)and non coronary disease group(n = 63).By the number of diseased vessels,we divided coronary disease group into single diseased group(n = 29)and mult diseased group(n = 55).Statistical software was used to analyze the differences of general clinical data,symmetrical-dynamic arteriosclerosis index,degree of coronary artery lesions,and the correlation between symmetrical-dynamic arteriosclerosis index and Gensini score.Results:(1)Comparison of general data between CHD group and non CHD group: smoking history,SCR,UA,Cys-C,Hb A1 c,Gensini score increased significantly(P < 0.01),HDL,ATIII decreased significantly(P < 0.01);(2)Comparison of ambulatory blood pressure parameters between CHD group and non CHD group:SBP,PP,MBP,D-SBP and N-SBP increased significantly(P < 0.01);(3)The level of S-AASI in CHD group was significantly higher than that in non CHD group(0.56 ± 0.07 vs 0.44 ± 0.08,P < 0.001);(4)Comparison of general data between single vessel disease group and multi vessel disease group: age,Hb A1 c increased(P < 0.05),Gensini score increased significantly(P <0.01),ATIII decreased(P < 0.05);(5)Comparison of ambulatory blood pressure parameters between single vessel disease group and multi vessel disease group: DBP and MBP were different(P < 0.05);(6)The level of S-AASI in multi vessel lesion group was higher than that in single vessel lesion group(0.65 ± 0.12 vs.0.70 ± 0.09,P = 0.044);(7)Correlation analysis of Gensini score: Cys-C(r = 0.319,P = 0.001),Hb A1c(r = 0.319,P < 0.001),PP(r = 0.297,P < 0.001),S-AASI(r = 0.599,P = 0.001)were significantly correlated with Gensini score,and positively correlated;ATIII(r =-0.289,P = 0.005)were significantly correlated with Gensini score,and negatively correlated.(8)By regression analysis we know high S-AASI score as a independent risk factor effect on the occurrence and development of premature coronary disease.(OR= 1.437,95%confidence interval: 1.034-1.999,P = 0.031)Conclusion:The level of S-AASI in patients with early-onset coronary heart disease was significantly increased,and increased with the increase of the severity of coronary artery disease and the number of involved branches.At the same time,the level of S-AASI in patients with early-onset coronary heart disease was significantly correlated with the severity of coronary artery disease,and the increased level of S-AASI was a risk factor for premature coronary disease.
