Research On The Operation Of General Practitioner Transfer Training In Xinjiang Township Health Centers

Objective: The training program for general practitioners in Xinjiang township hospitals was launched in 2010 and is expected to end in 2020.In this regard,this paper carried out a comprehensive and systematic study on the operation of the training program for general practitioners in Xinjiang Township Health Centers from the three-dimensional aspects including training process design,training staff size,and beneficiaries of the training project.This study aims not only to sum up and comb the project implementation for 10 years,it is also an empirical basis,and puts forward suggestions for the project’s trend after 2020.Methods: Using stratified random sampling and convenient sampling methods,47 township hospitals in 5 prefectures in Xinjiang were included in the sample institution.Using the census method,709 clinicians and 47 township hospital directors from 47 township hospitals were selected as the research subjects.Through questionnaire surveys and interview methods,from October 2019 to December 2019,709 questionnaires were filled out in 47 township health centers through face-to-face focused instruction and guidance.Forty-seven township health center directors were interviewed;this paper uses Epidata 3.0software to establish a database for official data and survey data;SPSS 21.0statistical software was used for data analysis and descriptive statistics on all data.In this paper,indicators such as composition ratio and mean±standard deviation are used;This paper used hypothesis testing,t-test,F-test,and chi-square test,regression and correlation analysis,and person correlation and multi-factor unconditional logistic regression analysis.Results:(1)The Xinjiang Uygur Autonomous Region Health Department formulated a training procedure for general practitioners in Xinjiang Township Health Centers at the end of 2010 based on the situation in Xinjiang.(2)From 2010 to 2019,a total of 1,358 GP transfer places were allocated to township health centers in 14 cities and towns in Xinjiang;an average of 1 to 2.5 GP transfer places were provided to every township health center in 12 cities in Xinjiang.In the past 10 years,the number of general practitioner training in township health centers accounted for 46.6% of the total number of general practitioner training in Xinjiang township health centers.(3)The total satisfaction score of 132 trainees who have participated in the transfer training for township health centers and the average satisfaction scores for the 10 dimensions are greater than 4.Overall,132 trainees who have participated in the transfer training are relatively satisfied and satisfied.There is a close correlation between satisfaction in each dimension and total satisfaction.Based on the sociological characteristics of the population,this paper analyzes the influencing factors for the overall satisfaction of the trainees who have participated in the transfer training,and the results showed that only working years had a significant impact on the overall training satisfaction,P<0.05.(4)After 132 trainees who have participated in the transfer training have been trained,their overall knowledge and skills and their mastery in the five dimensions are relatively improved,and they almost master the training projects.Their knowledge and skills mastery degree for each dimension are closely related with the overall mastery,and they are representative;based on demographic sociological characteristics,this paper analyzes the influencing factors of the overall mastery for knowledge and skills after training for trainees,and the results show no statistical significance,P>0.05.(5)132 trainees who have participated in the transfer are satisfied after the training in the overall knowledge and skills and the application of the five dimensions;and in terms of the order of application of the five dimensions,after training,the application of doctor-patient communication skills ranked first.The relationship between the application of knowledge and skills in each dimension and its overall application is closely related and representative;based on the sociological characteristics of the population,this paper analyzes the influencing factors of the overall application of knowledge and skills after the transfer trainers have participated in the training.The results showed no statistical significance,P>0.05.(6)Their mastery and application of knowledge and skills are consistent after the 132 transfer trainers received the training.(7)The interview results show that 43 of the 47 township hospital directors interviewed believe that the trainees have improved in basic medical knowledge,clinical skills,awareness of preventive health care and quality of medical services.The 47 directors pointed out that the number of staff who have participated in general practitioner transfer training are the same with the number of staff who have arrived in the position after the training,but there is a phenomenon of brain drain after training;45 directors believed that it is necessary to continue the implementation of the general practitioner transfer training project at the township hospital level;95.7% of the directors in the township hospitals thinks that the main difficulty is the lack of staff in the hospitals and the frequent secondment of staff;91.4% of the directors pointed out that competent medical staff could not be sent out,and no one was responsible for their original work during the training.(8)Of the 577 medical staff who did not participate in the general practitioner transfer training,492 expressed their willingness to participate in the general practitioner transfer training in township health centers;based on demographic sociological characteristics,this paper presents 577 medical staff who did not participate in the general practitioner transfer training and the influencing factors like the willingness to participate in general practitioner transfer training were analyzed,and the results showed that there were statistically significant differences in the willingness to participate in general practitioner transfer training among people with different ages,different educational backgrounds,and whether they were engaged in general practice,P<0.05.Conclusion: At the end of 2010,the Xinjiang Uygur Autonomous Region Health Department formulated the training procedure for general practitioner transfer in Xinjiang township health centers;from 2010 to 2019,the implementation of the training program for general practitioners in Xinjiang township hospitals has the highest contribution rate in the general practitioner transfer training in Xinjiang township hospitals;those who have participated in the transfer training are very satisfied with the general and 10 dimensions of the transfer general practitioner transfer training in township health centers.Their application ranks highly,and the mastery and application of knowledge and skills have been consistently enhanced after training;most township hospital directors recognize the effect of general practitioner transfer training and hope to continue to implement township hospital general practitioners transfer training project;most of the trainees who did not participate in the transfer training are willing to participate in the transfer general practitioner transfer training.
