The Study On The Chinese Medicine Treatment Regularity Of Lumbar Spinal Stenosis Based On Hospital Information System By Data Mining

Objective:Lumbar spinal stenosis(LSS)is a common clinical disease,it’s also the preponderant illness for TCM.This study adopted the method of data mining(DM)to analyze the hospital information system(HIS),for exploring the rule of CHMs and academic thoughts,summarizing the drug classification,core CHMs pairs and prescriptions.What’s more,this study provided reference for clinicians especially those who with low seniority,and recorded the experience of this disease scientifically.Methods:The diagnosis and treatment cases of LSS in Department of Orthopedics,Hubei Provincial Hospital of TCM between October 2016 and Octoberr 2018 were collected.This cases who were diagnosed strictly of LSS and treated with CHMs,were summarized as 178 valid prescriptions.The data were extracted after standardization,sorted out and summarized in Excel,then the database was established.According to the data of the database,the CHMs’ frequency was sorted by Excel,and the classification was analyzed.IBM SPSS Modeler 18.0 was used for the association analysis of core-CHMs pairs and groups.IBM SPSS Statistics 21.0 was adopted for clustering analysis,and exploring the relationships within CHMs.Results:1.The Frequency of CHMs:In the entry of 93 CHMs,48 CHMs havehigher frequency than 10,the totality is 1596,which accounts for 88.18% of all.The top 15 CHMs are in the order:eucommia ulmoides,angelica sinensis,licorice root,achyranthes bidentata,deerhorn glue,rehmannia rehmanniae,radix paeoniae alba,astragalus membranaceus,ligusticum chuanxiong,dipterus dipsacus,centipede,radix paeoniae rubra,rhizoma cibotii,scorpion.The frequency of above CHMs are equal or greater than 39,which are commonly used for treating the disease.2.Classification of CHMs:After the standardized classification,a total of14 CHMs categories came out.According to the frequency(percentage)of each categorie,each type is in the order:Restorative CHMs(34.97%),CHMs for Invigorating Circulation and Removing Blood Stasis(25.14%),antirheumatic(15.25%),CHMs for Calming the Liver and Stopping Endogenous Wind(7.35%),CHMs for Relieving Exterior Syndromes(4.09%),CHMs for Clearing heat and expectoration(2.71%),Warming Interior CHMs(2.54%),CHMs for Regulating Qi(1.93%),Phlegm Resolving,Antitussive and antiasthmatic CHMs(1.82%),Astringent(1.55%),CHMs for Clearing Interior Heat(1.10%),Hemostatic CHMs(0.94%),Cathartic CHMs(0.50%),Tranquillizers(0.11%).3.Correlation Analysis:Eligible prescription were analyzed(set up the’confidence degree’ was 80% or higher,’support degree’ was 10% or higher),and ordered by frequency of the former rules 46.The top 3 rules are:’eucommia ulmoides-angelica sinensis’ ’Angelica sinensis-Achyranthes bidentata’’Angelica sinensis-radix paeoniae alba’.According to the lift degree,the rules of the top 3 are:’cortex acanthopanacis,Achyranthes bidentata,pericarpium citri reticulatae viride’’dogwood,prepared rehmannia root,Achyranthes bidentata,pericarpium citri reticulatae viride’’dogwood,prepared rehmannia root,Achyranthesbidentata,Eucommia ulmoides,pericarpium citri reticulatae viride’.4.Clustering Analysis:There are five groups of Restorative CHMs,five groups of CHMs for Invigorating Circulation and Removing Blood Stasis and four groups of Antirheumatic.Conclusions:1.’Eucommia ulmoides,angelica sinensis,licorice root,achyranthes bidentata,deerhorn glue,rehmannia rehmanniae,radix paeoniae alba,astragalus membranaceus,ligusticum chuanxiong,dipterus dipsacus,centipede,radix paeoniae rubra,rhizoma cibotii,scorpion’are the core single drugs for the treatment of LSS.2.’Restorative CHMs,CHMs for Invigorating Circulation and Removing Blood Stasis,antirheumatic,CHMs for Calming the Liver and Stopping Endogenous Wind,CHMs for Relieving Exterior Syndromes,CHMs for Clearing heat and expectoration,Warming Interior CHMs,CHMs for Regulating Qi,Phlegm Resolving,Antitussive and Antiasthmatic CHMs,Astringent,CHMs for Clearing Interior Heat,Hemostatic CHMs,Cathartic CHMs,Tranquillizers’ are common CHMs classification for treating LSS.3.’Eucommia ulmoides-Angelica sinensis,Angelica sinensis-Achyranthes bidentata,Angelica sinensis-radix paeoniae alba’are common CHMs pairs for the treatment of LSS.4.’Dogwood,prepared rehmannia root,achyranthes bidentata,pericarpium citri reticulatae viride’’dogwood,prepared rehmannia root,Achyranthes bidentata,Eucommia ulmoides,pericarpium citri reticulatae viride’’prepared rehmannia root,radix paeoniae alba,achyranthes bidentata,pericarpium citri reticulatae viride’’saposhnicovia divaricata,radix angelicae pubescentis,pa rasitic loranthus,asarum achyranthes bidentata,angelica sinensis,Eucommia ulmoides,asarum’ are common CHMs groups for the treatment of lumbar spinal stenosis.5.’Nourishing liver and kidney,ehumidification,promoting qi to activate blood and invigorating qi and promoting blood circulation’are the basic treatment for lumbar spinal stenosis.6.The Experience of taking CHMs implys ’TaorenHonghuadecoction’,’Duhuojisheng decoction’and the other famous prescriptions.Eucommia ulmoides,dog ridge and the other have special and important effects.
