| Objective:Through the collection of clinical data of 100 cases of Sanders III calcaneal fractures,retrospective analysis of traditional Chinese medicine revised Taohong Siwu Decoction in the calcaneal fracture incision and the efficacy and safety of fixation before and after reduction and internal,the assessment revised whether Taohong Siwu Decoction can promoting fracture healing,reducing injury after foot swelling and reducing postoperative incision edge necrosis,infection rate.And provide the basis for clinical.Method:100 cases of Sanders III calcaneal fractures were treated in Hubei Provincial Traditional Chinese Medical Hospital from October 2010 to October 2015,which were treated with open reduction and internal fixation of calcaneal fractures in Hubei Provincial Traditional Chinese Medical Hospital.The cases were divided into two groups according to the treatment.After admission,all patients were elevated their affected limbs,plaster fixation for symptomatic treatment,and the treatment was given 3 days before and 4 days after the operation.The treatment group was treated with oral addition and Taohong Siwu Decoction,while the control group was treated with intravenous infusion of seven sodium saponin.After statistical analysis of the types,age and sex ratio of the two groups,it was found that the difference was not statistically significant(p>0.05).1 weeks before and after operation,2 weeks after operation,3 months after discharge,12 months after operation,and 18 months after operation,compared the injured foot swelling rate and postoperative infection cases,preoperative and postoperative B?hler′s angle(calcaneus joint angle),Gissane’s angle(cross angle,angle of calcaneal fractures).The healing rate of incision skin,the incidence of incison margin necrosis and Maryland foot Score after operation were compared.Result:All 100 patients were followed up at 18 months after operation,and the average follow-up time was(18.20 ± 2.15)months.No fracture nonunion was found in all follow-up patients after reexamination.In the time of skin wrinkles(after injury to operation time),the average time of oral Chinese medicine group was(4.90 ± 0.15)d,and the average time of the intravenous infusion of seven sodium saponin was(5.23 ± 0.28)d,and the difference between the two groups was statistically significant(P<0.05).There were 2 cases of skin necrosis in the oral chinese medicine group,1 cases with superficial tissue infection,no deep tissue infection occurs,adverse skin healing rate was 6%;5 cases of intravenous infusion of seven sodium saponin,2 cases of superficial tissue infection,no deep tissue infection.adverse skin healing rate was 14%;two groups of adverse skin healing rate was not statistically significant(P>0.05).To collect image data in all patients before surgery and 12 months after treatment,measured and analyzed;B?hler′sangle: Chinese medicine group before operation,the average value is 8.28 ± 2.59,after operation is an average of 30.19 ± 1.78;intravenous infusion of seven sodium saponin group before operation,the average value is 7.56 ± 2.11,check after an average of 30.88 ± 1.98.Calcaneus Gissane ’s angle: the average value of the oral Chinese medicine group before operation was 83.13 ± 1.06,the average value after checking was 120.68 ± 1.78,and the average value of intravenous infusion of seven sodium saponin sodium group was 81.47 ± 1.89.The average value of the reexamination was 131.65 ± 1.56.The excellent and good rate of the two groups: the excellent and good rate of oral Chinese medicine group was 94%;the excellent and good rate of intravenous infusion of seven sodium saponin sodium group was 92%.After analyzing the B?hler’s,Gissane’ angle,and the excellent and good rate of the foot function before and after the two groups,it was found that the difference was not statistically significant(p>0.05).Conclusion:It was found that there was no significant difference in the healing time between the two groups in the excellent rate of limb function and B?hler’s or Gissane’ s angle(p>0.05).There was a significant difference between the two groups in the rate of swelling of the feet and the incidence of necrosis and infection on the edge of incision(P<0.05).It shows that the decoction of Taohong Siwu has obvious effect on the decline of the swelling of the affected limbs after the fracture of Sanders III calcaneal fractures and the prevention of the necrosis of the skin margin and the infection after internal fixation. |
