Objectives:Air force pilots are an important embodiment of our army's air combat capabilities.Because of long-term special work and training surroundings,physical health level acts as an important security of fighting capacity for them.Recently,we conducted a survey of oral health status for air force pilots who arrived in an aviation medical appraisal and training center for health recuperation,and obtained the status of oral health behavior and oral health disease for these air force pilots on active duty.By setting the adult molar fissure classification standard,the air force pilots' pit and fissure status of post-molars were investigated,analyzed,and studied.By investigation,we analyzed the current problems of oral health status and put forward development countermeasures from aspects of grassroots units of the air force,department of stomatology of aviation medical appraisal and training centers,and oral health administration of air force.Based on the investigation and analysis of oral health status of the air force pilots,we conducted an innovative andpreliminary study of post molars' pit and fissure status,and provided the evidence to the need of pit and fissure sealant application for air force pilots.Methods:By stratified cluster sampling method,released by the WHO oral health survey method as the basis,and the army oral health survey plan(2013)for the reference to design the oral disease investigation form of air force pilots,we took the air force pilots who were early arrived in the aviation medical appraisal and training center from each period with the proportion of 50% as the research objects to conduct the survey of oral health.The survey was conducted by two practicing dentists,who were trained in standard training and standard consistency test.The Kappa value was from 0.6 to 0.8.Understanding through investigation purpose,survey method and survey standard was unified before field investigation,and the investigation process was developed and finished under the instruction of teachers in department of preventive dentistry of The Fourth Military Medical University Stomatology Hospital.Investigation scene was in the stomatology department under the Physical Examination Dept.of the center.The survey instruments were disposable stomatoscope,tweezer,probe,and periodontal probe.High-temperature sterilization was conducted before each use.Respondents received oral examination in the dental treatment seat with LED light.The investigation results were recorded in investigation forms.The oral health questionnaire and oral health examination form collected in scene were input in the database via FoxPro 6.0 software,and we used SPSS19.0 software to perform statistical description and statistical analysis.Results:Total survey portion of the are force pilots was 172,all for men,ages from 22 to55,32.20±1.06 years of age.Men within 30 years old were 90,accounting for 52.3%,and men over 30 years old were 82,accounting for 47.7%.According to born sources,men from urban areas were 75,accounting for 43.6%,and men from rural areas were 97,accounting for56.4%.According to the service age,men with service age over 8 years were78,accounting for 45.3%,and men with service age within 8 years were 94,accounting for54.7%.In oral health behavior questionnaire survey,the daily brushing rate was 100%.The rate of brushing teeth 2 or more times a day was 76.7%,and respondents who brushed teeth one time a day accounted for 23.3%.The rate of having used floss was 26.7%.The mouthwash utilization rate was 23.3%.The fluoridated toothpaste utilization rate was36.6%.Respondents who had used toothpick accounted for 65.7%.Over the past year,respondents who didn't have gingival bleeding accounted for 25.0%,63.4% for gingival bleeding sometimes,and 11.6% for gingival bleeding often.When gingival bleeding happened,the rate of behavior selection from high to low was washing mouth with water(42.4%),paying attention to brushing(34.3%),neglect(26.2%),washing mouth with mouthwash(7.0%),consulting the doctor(4.1%),and medication(2.9%).Within a year,there were 52.9% respondents had hospital oral examination or treatment,and 15.1%for who had attended hospital oral emergency treatment.The rate of respondents who knew himself had tight-teeth in flight was 14.5%,62.8% for respondents who didn't have,and 22.7% for who wasn't clear about it.In oral disease survey,the caries prevalence rate was 67.4%,and mean dmft was1.31.The number of teeth that need treatment because of caries accounted for 47.9% in the teeth that decayed or fixed.The periodontal health rate was 16.3%.The detection rate of gingival bleeding was 64.5%,85.5% for dental calculus,and 33.7% for periodontal pocket,11.0% for wedge-shaped defect,and 44.2% for pathological tooth wear.In the depth of post-molars' pit and fissure survey,the detection rate of respondents who had deep pit and fissure in post-molars was 43.0%.The detection rate of post-molars with deep pit and fissure,tooth 16,17,26,27,36,37,46,47 was 3.0%,7.7%,3.0%,6.6%,16.0%,27.3%,14.2%,31.1%.The detection rate of deep pit and fissure of the mandibular second molar was the highest,the mandibular first molar ranked the middle,and the maxillary first molar was the lowest.The difference among these three rates showed statistically significant.The post-molar's pit and fissure morphology investigation:The adult's occlusive surface of post-molars is prone to pit and fissure caries.The third molar impactionsurvey:37.8% for third molar impaction rate,and proximal or distal impaction rate was the highest among impacted third molars,which was 57.6%.The mandibular third molars were more compared with the maxillary,which was 80.8%.Conclusions:Oral health status of air force pilots:The concept of oral health in the grassroots of air force is weak,and there exists some misunderstandings upon oral health behavior.Department of stomatology of air force aviation medical appraisal and training center needs to improve the flexibility of oral service,and the normalization should be strengthened.The air force oral health service support system needs to be strengthened.The condition that air force personnel obtaining oral health guidance has been improved.The early prevention of oral disease and the oral health maintenance for air force pilots should be paid more attention,and to further establish and improve the oral health care system for air force pilots.The current situation of oral disease prevention in our air force pilots need to be improved.There is a considerable part of deep pit and fissure among air force pilots' post-molars,and the proportions vary in different dental positions,which provides evidence to predict high risk dental positions of caries to air force pilots.Air force pilots' environmental closure is obvious,and not easy to get sufficient access to timely oral consult.Bringing pit and fissure sealing treatment into air force pilot's oral health nursing system can reduce the risk of caries to them,and has great significance in the prevention of oral diseases.
