Objective: To explore the impac of Type D personality on depression and quality oflife of patients with atrial fibrillation(AF);and to determine whether Type Dpersonality and depression were independently associated with impaired quality oflife in AF patients,which would provide theoretical basis of psychological behaviorintervention for AF patients.Methods: A cross-sectional study design was used among a total of156patients withchronic AF(persistent or permanent), and they had at most mild valvular disease.Patients were excluded from this study if they had organic heart disease and hadNYHA class IV.The participants were divided into two groups according to DS14(Type D Personality Scale)scores,and all of them were investigated with the BeckDepression Inventory(BDI), the MOS item short form health survey(SF-36)within oneweek since admitted. Statistical analyses were performed with SPSS18.0.Results:151valid questionnaires were obtained including92from males and59from females, the mean age (57.5±11.4)year.(1)31.8-percent of patients had Type D personality(48/151)while68.2-percent hadNon-D personality(103/151). There were significant differences between two groupsin comorbidity(Hypertension, Heart failure). And other demographic and clinicalcharacteristics were no significant difference.(2) The constituent ratio of Type D patients with depression was significantlydifferent from Non-type D patients(37.5%vs17.48%,p=0.007),and patients withType D personality had higher BDI mean than those with non-Type D personality(14.3±12.2vs.10.0±8.8, p=0.042).(3) Univariate logistic regression analysis of each variable for prediction of AF patients with depression showed: OR=0.656(95%CI:0.264-1.210) for educationallevel; OR=0.265(95%CI:0.185~1.160) for sex;OR=3.494(95%CI:1.191-7.090) forType D personality. Multivariate logistic regression analysis showed only Type Dpersonality had the role in the prediction of AF patients with depression([OR:3.264(95%CI:1.076~5.489),P <0.01].(4) The scores of role-physical(RP), general health(GH),vitality(VT). socialfunctioning(SF),role-emotional(RE), mental health(MH) and SF-36in patients withtype D personality were lower than the patients with non-type D personality (p<0.05or0.01)except bodily pain(BP) and PF. Type D personality, depression had negativecorrelation with the quality of life of AF patients (r=-0.289,-0.453, p<0.01), buteducational level and sex had positive correlation with the quality of life of AFpatients (r=0.356,0.154, separately, p<0.01). Multiple stepwise regression analysisshowed that depression, type D personality,and education level were entered QOLregression equation (β=0.06,-0.498,0.072, p<0.05or0.01).Conclusion:1. Type D personality is a risk factor for AF patients accompanied withdepression.2. Type D personality is a independent predictor of impaired HRQoL ofAF patients.3. Medical staff need to pay attention to patients with Type D personalityto prevent impaired HRQoL in AF patients.
