Objective:To observe the analgesia effect of postoperative using bupivacainecombined with fentanyl through sacral canal in pediatric operation on abdominalregion. And discuss the safety and practicality.Methods: Eighty children undergoing selective abdominal operation wererandomly assigned of four groups by using different kinds of drugs, twentychildren in every group. Group A used0.1%bupivacaine combined with0.1μg/kg fentany through sacral canal postoperatively, Group B used0.1%bupivacaine through sacral canal postoperatively, Group C used0.1μg/kgfentany through sacral canal postoperatively, Group D used nothing, volumeof drug is1.0ml/kg. Record the systolic blood pressure(SBP)、diastolic bloodpressure(DBP)、respiratory rate(RR)、 heart rate(HR)and saturation of bloodoxygen(SpO2) at different time of every group, and record the duration ofsurgery, the duration of analgesia and the hospital stay. Record the FLACC scoreand the adverse reaction. Results: There was no statistics difference in sexuality、age、weight、the type of surgery、the duration of surgery、the durationof analgesia and the hospital stay(P>0.05). The FLACC scores in group A、B、C were significantly lower than group D at2h after the operation(P<0.05),The FLACC scores in group A、B were significantly lower than group C、D at4hafter the operation(P<0.05),The FLACC scores in group A were significantlylower than group B、C、D at8h after the operation(P<0.05), The FLACCscores at12h、24h、48h after the operation have no difference among group A、B、C、D (P>0.05). the FLACC scores of group A、B、C after operation waslower than extubation(P<0.05).the HR in group A was significantly slowerthan group B、C、D(P<0.05)at the time of extubation、30min after extubation、2h、4h、8h12h after the operation.And there was no statistics difference in SBP,DBP, RR and SpO2between the two groups. Each group was occasionally foundpostoperative nausea and vomiting, itchy skin and other adverse reaction, eachgroup was not found urinary retention, sports block and respiratory inhibition.Conclusion: single using bupivacaine or fentanyl through sacral canal used forpostoperative analgesia of infants,abdominal surgery can provide certainanalgesic effect, and adverse reaction does not increase, it is a safe and effective,simple and practical demulcent method, is worthy to be popularized.
