Objectives:Starting from the autonomic nerve dysfunction in the recovery period of stroke,using heart rate variability assessment technology,through the collection and analysis of HRV test data,and the clinical feature scale(Clinical Feature Scale,CFS)to further clarify that acupuncture has different effects on the recovery period of stroke The therapeutic effect of severe autonomic dysfunction.Explore the feasibility and effect of clinical acupuncture therapy for autonomic nerve dysfunction after stroke,and realize its application value in clinical medicine.Methods:This trial adopts a randomized,single-blind,and controlled method,strictly in accordance with the criteria of the trial design,and was admitted to the rehabilitation department of Tianshan Hospital of Traditional Chinese Medicine in Changning District,Shanghai from May 2020 to January 2021 and was diagnosed with stroke recovery.Sixty-four cases of autonomic dysfunction were taken as the research objects.In this experiment,all subjects were divided into 1 group(treatment group)according to the principle of random allocation,that is,32 people in the acupuncture group,and 32 people in the second group(control group).All subjects can use basic symptomatic treatment drugs,such as antihypertensive drugs,lipid-lowering drugs,hypoglycemic drugs,antiplatelet aggregation drugs,etc.In the treatment group,acupuncture intervention was performed on the basis of symptomatic treatment drugs,and clinical observation was conducted for 4 weeks.Acupuncture points on both sides of Neiguan and Zusanli on both sides.Leave the needle for 20 minutes after the acupuncture is inflated.Acupuncture treatment is performed every other day,3 times a week,12 times(4 weeks)as a course of treatment.The control group was clinically observed for 4weeks on the basis of symptomatic treatment drugs.Collect baseline(W0)and 4 weeks of treatment(W4)data.Before treatment and 4 weeks of treatment,patients were tested for 24 h HRV and CFS score and recorded in detail.Statistical analysis was performed using SPSS22.0 to objectively evaluate the clinical effects of acupuncture on 24 h HRV related indicators and CFS scores in improving autonomic dysfunction in patients with stroke recovery,and to evaluate its safety.Results:Case completion: A total of 64 patients with autonomic nervous dysfunction in the convalescent stage of stroke were admitted in this study.During the process,a total of 4patients were terminated and classified as abjection cases,so a total of 60 patients completed the study.1.Baseline study: Before treatment,all indicators of the groups maintained good homogeneity,and the baseline was balanced.For the age,gender and body mass index(BMI)of the subjects were statistically analyzed,and the results showed P > 0.05,which means the difference was not statistically significant,and there were clinical comparability between the two groups.2.Comparison within the treatment group: The 24 h HRV results of the treatment group after 4 weeks of treatment showed that SDNN,RMSSD,LF,HF,LF/HF were higher than before treatment,the difference was statistically significant(P<0.05),SDANN,PNN50 The difference from before treatment was not statistically significant(P>0.05).The CFS score of the treatment group was lower than before treatment after 4weeks of treatment,the difference was statistically significant(P<0.05).3.Comparison within the control group: The 24 h HRV results of the control group after 4 weeks of treatment showed that SDNN,RMSSD,LF,HF were higher than before treatment,the difference was statistically significant(P<0.05);SDANN,PNN50,LF/HF The difference from before treatment was not statistically significant(P>0.05).The CFS score of the control group was lower than before treatment after 4 weeks of treatment,and the difference was statistically significant(P<0.05).4.Comparison between the treatment group and the control group: 24 h HRV results showed that compared with the control group,there was no statistically significant difference in SDNN,SDANN,RMSSD,PNN50,LF,HF,LF/HF between the two groups before treatment(P>0.05).After 4 weeks of treatment,the levels of SDNN,RMSSD,LF,HF,and LF/HF in the treatment group were higher than those in the control group,and the difference was statistically significant(P<0.05).The levels of SDANN and PNN50 in the treatment group were not significantly higher than those in the control group,That is,the difference is not significant(P>0.05).From the CFS score,the difference in CFS scores between the two groups before treatment was not statistically significant(P>0.05).After 4 weeks of treatment,the CFS score of the treatment group was lower than that of the control group.5.Comparison of total effective rate: The total effective rate of the treatment group is 93.33%,and the total effective rate of the control group is 73.33%.And after the χ2 test,χ2=4.320,P<0.05,the difference between the effective rates of the two groups of data is statistically significant and clinically comparable,so it can be inferred that the treatment group is better than the control group.in conclusion:1.Both the treatment group and the control group can increase the SDNN value of patients with autonomic dysfunction during stroke recovery,and improve the overall autonomic activity of the patients,and the effect of acupuncture treatment is more significant.2.Both the treatment group and the control group can increase the LF value of patients with autonomic dysfunction during stroke recovery,and increase the sympathetic nerve activity of the patients,and the effect of acupuncture treatment is more significant.3.Both the treatment group and the control group can increase the RMSSD and HF values of patients with autonomic dysfunction during stroke recovery,and increase the parasympathetic activity of the patients,and the effect of acupuncture treatment is more significant.4.Acupuncture treatment can increase the LF/HF value of patients with autonomic dysfunction in the recovery period of stroke,and can adjust the balance of autonomic nerves in patients.5.Both the treatment group and the control group can reduce the CFS score of patients with autonomic dysfunction during stroke recovery period,which is beneficial to the recovery of autonomic nerve function of patients,and the effect of acupuncture treatment is more significant.6.The incidence of adverse reactions during the trial was 1.56%,and the overall safety was good.
