Purpose:To observe the curative effect of TCM fumigation on wound healing after damp-heat anal leakage.Material and method:A total of 60 patients diagnosed as damp-heat diathermy anal leakage in the anorectal Department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from November 2021 to October 2022 who received line hanging treatment were included according to the inclusion and exclusion criteria.Using random number table method,60 patients were randomly divided into stage fumigation group and ordinary fumigation group,with 30 cases in each group.The staging criteria refer to the9th edition of Pathology[1]and Expert Consensus on Management Norms for Wound Repair after anorectal Surgery(2019)[2]:inflammation stage,granulation tissue growth stage and epithelial growth stage are divided into three stages.In the stage fumigation group,at the early stage(1-3 days after surgery),fumigation I prescription plus honeysuckle and dandelion fumigation was applied;In the middle stage(4-7 days after surgery),fumigation prescription I was used to add blood exhaustion and frankincense fumigation.In the later stage(8-14 days after surgery),fumigation prescription I plus astragalus and angelica were applied,and fumigation prescription I was directly applied to the common fumigation group.Conventional postoperative treatment was used in both groups.Postoperative wound pain,anal edema,wound secretion exudation,wound granulation tissue morphology,wound area healing rate and wound healing time of the two groups were recorded,and the data were input into IBM SPSS Statistics 26.0 for statistical analysis.Results:1.In terms of wound pain scores at 3,5 and 7 days after surgery,the staging fumigation group had a significant advantage over the common fumigation group in relieving postoperative pain,and there was a statistical difference between the two groups(P<0.05);There was no statistically significant difference between the two groups on day 1 and 14 after surgery(P>0.05).2.In terms of the degree of anal skin edema at 3,5 and 7 days after operation,the staging fumigation group was better than the ordinary fumigation group,and there was a statistical difference between the two groups(P<0.05).14 days after surgery,there was no significant difference between the two groups(P>0.05).3.In terms of wound exudation at 3,5 and 7 days after surgery,the staging fumigation group was better than the ordinary fumigation group,and there was a statistical difference between the two groups(P<0.05).14 days after surgery,there was no significant difference between the two groups(P>0.05).4.At 5,7,14 days after surgery,granulation tissue morphology of the two groups was accelerated in the staging fumigation group compared with the ordinary fumigation group,and there was a statistical difference between the two groups(P<0.05).On the 3rd day after surgery,there was no significant difference between the two groups(P>0.05).5.In terms of postoperative wound healing time between the two groups,the staging fumigation group could shorten the wound healing time compared with the common fumigation group,and there was a statistical difference between the two groups(P<0.05).6.In terms of wound area healing rate 7 and 14 days after surgery,the staging fumigation group could accelerate wound healing compared with the ordinary fumigation group,and there was a statistical difference between the two groups(P<0.05).7.No adverse reactions occurred in the process of postoperative fumigation and dressing change in the two groups,which is worthy of further promotion.Conclusion:1.Both the stage fumigation group and the ordinary fumigation group were effective in wound healing after anal leakage.2.In terms of reducing wound pain score,anal border edema score,promoting granulation tissue growth,reducing secretion exudation,accelerating wound healing and shortening wound healing time,the curative effect of staging fumigation group was better than that of ordinary fumigation group.
