Clinical Study On Fang's Scalp Acupuncture Combined With Vestibular Rehabilitation In The Treatment Of Persistent Postural-perceptual Dizzines

Purpose:To explore the clinical efficacy,possible mechanism and safety of Fangshi scalp acupuncture combined with vestibular rehabilitation in the treatment of persistent postural and perceptual dizziness.Materials and methods:93 patients with PPPD who were admitted to the Department of Otolaryngology,Head and Neck,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from August 2020 to January 2023 were selected,and they were required to meet the inclusion criteria and exclusion criteria.The patients were randomly divided into control group,acupuncture group and combined group,with 31 cases each.All the patients in the three groups were given oral treatment of Routai.The acupuncture group was also treated with Fang’s scalp acupuncture.The combination group was treated with vestibular rehabilitation on the basis of acupuncture group.All patients in the three groups received routine health education,and the treatment period was 4 weeks.Before and after treatment,the vertigo and disability scale score,anxiety and depression scale score,Pittsburgh sleep quality index score,P300 latency and amplitude,and clinical efficacy evaluation were evaluated respectively.SPSS 26.0 software was used to statistically process the data obtained by using single factor sample t test,paired sample t test,nonparametric test,and chi-square test,P<0.05 indicates statistical significance.Result:1.Before treatment,there was no statistical difference in the general data and observation indicators of the three groups of patients(P>0.05).2.DHI score:After treatment,the scores of DHI-P,DHI-E,DHI-F and total DHI in the three groups were lower than those before treatment(P<0.05).Compared with the acupuncture group,the DHI-P score and DHI-F score of patients in the combination group decreased more significantly(P<0.05);There was no difference in DHI-E scores between the two groups(P=0.095).3.HADS score:After treatment,compared with before treatment,the HADS score of patients in the three groups was lower than that before treatment(P<0.05).Compared with acupuncture group,there was no difference in the score of HADS between the two groups(P=0.137).4.PSQI score:After treatment,compared with before treatment,the PSQI score of patients in the three groups decreased significantly(P>0.05).Compared with the acupuncture group,the PSQI score of patients in the combination group decreased significantly(P<0.05).5.Event-related potential P300:After treatment,compared with before treatment,the P300 latency of patients in the three groups decreased(P<0.05).Compared with the acupuncture group,the P300 latency of patients in the combination group has no difference,and there is no statistical difference(P=0.252).The P300 amplitude of patients in the combination group has significantly increased,and there is a statistical difference(P>0.05).6.Clinical treatment effect:The total effective rates of the control group,acupuncture group and combination group were 76.67%,89.56%and 100%respectively,with statistically significant difference(?~2=6.11,P=0.047<0.05).Conclusion:1.Both scalp acupuncture therapy and Fangshi scalp acupuncture combined with vestibular rehabilitation can treat PPPD patients,which has a good effect on improving the quality of life of PPPD patients,reducing the degree of dizziness of PPPD patients,improving the quality of sleep of PPPD patients,enhancing the patients’cognition and reducing the anxiety and depression of PPPD patients.2.The overall treatment effect of Fangshi scalp acupuncture combined with VRT on PPPD patients is better than that of Fangshi scalp acupuncture alone,especially in improving the quality of life of PPPD patients,reducing the degree of dizziness of PPPD patients,improving the quality of sleep of PPPD patients,and reducing the anxiety and depression symptoms of PPPD patients.3.The treatment method of Fang’s scalp acupuncture combined with VRT is more specific in the treatment of patients with PPPD than the conventional simple medication or acupuncture treatment.It is safe,convenient and reliable,and can be promoted.
