| Objective:In this study,we investigated the current situation of supportive care needs in patients with lung cancer treated with immune checkpoint inhibitors,explored the correlation between supportive care needs and disease perception and social support,determined the influencing factors of patients’ supportive care needs,and provided theoretical basis for clinical medical staff to reduce unmet needs and formulate corresponding intervention measures.Methods:In this study,135 patients with lung cancer treated in the medical oncology department of a Grade A hospital in Jiaxing from September 2021 to July 2022 were selected as the convenience sample,Through the general data questionnaire,the comprehensive needs assessment scale of cancer patients(Comprehensive Needs Assessment Tool in cancer for patients,CNAT),concise disease perception questionnaire(The Brief illness Perception Questionnaire,BIPQ),Social Support Rating Scale(Social Supportive Rate Scale,SSRS)conducted the questionnaire survey.After data collection,the general data were described statistically by using mean,standard deviation,frequency and composition ratio;independent sample t-test and one-way variance analysis;Pearson correlation analysis to demonstrate the correlation between supportive care needs,disease perception and social support of lung cancer patients;and multiple linear regression method.Results:1.The dimensions of lung cancer patients were divided in descending order:medical needs(1.28±0.91),knowledge information needs(0.85±0.75),hospital facilities(0.75±0.69),nursing care(0.70±0.80),physical symptoms(0.52±0.80),actual support(0.43±0.58),psychological and emotional needs(0.40±0.68),and religious spiritual support(0.08±0.39).Among the top ten items with the highest supportive care demand score,4 were from knowledge information demand dimension,4 from medical demand dimension,1 from actual support demand dimension and 1 from hospital facility demand dimension.2.The results of univariate analysis showed that general data such as occupational status,education level,major economic source,health insurance,primary caregivers,immunotherapy courses and whether ir AEs occurred were statistically different in the scores of supportive care needs of lung cancer patients with ICIs,p<0.05.3.The mean disease perception score of immune checkpoint inhibitor-treated patients with lung cancer was 43.78±7.645,and the mean social support score was31.77±6.10.4.The results of the correlation analysis showed that the total score of supportive care needs was positively associated with disease perception(r=0.354,p<0.01);the total score of supportive care needs was negatively associated with social support(r=-0.237,p <0.01);and the disease perception with social support(r=-0.254,p <0.01).5.Regression analysis showed that occupational status(β =-7.917,p <0.05),major economic source(β =-1.125,p <0.05),immunotherapy course(β =-6.958,p<0.05),ir AEs(β = 6.673,p <0.05),disease perception(β = 0.151,p <0.05),and social support(β =-0.188,p <0.05)were factors influencing patients’ supportive care needs.The results of the regression analysis showed that occupational status,immunotherapy course,immune-related adverse reactions(ir AEs),disease perception,and social support were the influencing factors for patients’ supportive care needs.Conclusion:1.The supportive care needs of patients with lung cancer treated by immune checkpoint inhibitors were at mild to moderate levels,with the highest medical needs dimension score,followed by knowledge information needs and medical facility needs.2.Higher levels of disease perception and lower levels of social support may lead to higher levels of supportive care needs in lung cancer patients treated with immune checkpoint inhibitors.Clinical medical staff can provide patients and caregivers with health education and provide patients with more immune-related adverse reactions and management countermeasures,so as to reduce the unmet supportive care needs of patients and improve the quality of life of patients.3.The supportive care needs of lung cancer patients are mainly influenced by occupational status,major economic sources,immunotherapy courses,ir AEs,disease perception,and social support.Medical staff should focus on patients with different occupations,financial sources,immunotherapy within 3 courses,more ir AEs system involvement,higher disease perception,and lower social support. |
