Objective:Acute ischemic stroke(AIS)is an acute and severe disease with high disability rate due to cerebral ischemia and hypoxia.AIS vascular revascularization is currently the preferred treatment for cerebral infarction during the clinical window period,among which intravenous thrombolysis is the most widely used in clinical practice due to its safety and applicability.Exploring the physiological and pathological mechanism of this treatment to further improve clinical efficacy and improve patient prognosis has increasingly become a research hotspot in the medical field.Complications such as ischemia reperfusion and hemorrhage transformation after revascularization,resulting in enlargement of cerebral infarction area and even cerebral hemorrhage seriously affect the prognosis of patients,so it is extremely necessary to conduct drug intervention in the early stage after revascularization.At present,Western medicine usually lacks safe and effective drugs to prevent vascular circulation disorders.Traditional Chinese medicine injection can give full play to the advantages of traditional Chinese medicine,which is convenient,safe and effective,and basically covers the clinical treatment of AIS in China.Salvia miltiorrhiza polyphenol acid injection is an effective component extracted from Salvia miltiorrhiza,which can improve the cardio-cerebral blood system and alleviate ischemic reperfusion injury,and has been widely used in clinic.In this study,a rat model of cerebral artery embolization with allograft thrombosis was established,and the model rats were taken as the research objects to observe the effects of salvianolic acid early intervention on the expression of eNOS,sTM and sEPCR in rats with acute ischemic cerebral infarction.From the perspective of the protective effect of vascular endothelial function,To investigate the effect of salvianolic acid injection on cerebral artery circulation in early intervention of vascular recanalization,fill the current clinical research gap in the early treatment of vascular recanalization by promoting blood circulation and removing blood stasis,and provide experimental basis for clinical application.Methods:120 male SD rats were randomly divided into sham operation group,model group,thrombolysis group and salvianolic acid group,with 30 rats in each group.Each group was given different pretreatment.Rats in sham operation group and model group were injected daily with sodium chloride solution through tail vein.The rats in the thrombolytic group were injected with alteplase into the tail vein 4.5 hours after the completion of modeling,and were treated with equal volume of sodium chloride solution 2 hours later and every day.The salvianolic acid group was intravenously injected with alteplase 4.5h after the modeling was completed,and the salvianolic acid group was intravenously injected 2 hours later and daily.After modeling and administration,the mortality rate of the rats was counted,and the neurological defect behavior of the rats in each group was observed at 1d,3d and 7d after infarction according to the requirements.Brain tissue was retained for TTC staining to observe cerebral infarction lesions and size,HE staining to observe brain cell conditions,and immunohistochemical detection of eNOS,sTM and sEPCR protein expression levels in each group.The data obtained were analyzed by statistics.Results:1.Overall mortality of rats in each group:compared with the sham operation group,the mortality of model group and thrombolytic group was significantly increased,and the mortality of thrombolytic+salvianolic acid group was increased.2.Comparison of neurological impairment scores among all groups:1 day after modeling:the neurobehavioral scores of model group,thrombolytic group and thrombolytic+salviorrhiza group were higher than those of sham operation group.After 3 days of modeling,the neurobehavioral scores of model group,thrombolytic group and thrombolytic+salvia miltiorrhiza group were higher than those of sham operation group,while the neurobehavioral scores of thrombolytic group and thrombolytic+salvia Miltiorrhiza group were lower than those of model group.After 7 days of modeling,the neurobehavioral scores of the thrombolytic group and the thrombolytic+salvia miltiorrhiza group were higher than those of the model group,and the neurobehavioral scores of the thrombolytic group were higher than those of the thrombolytic+salvia Miltiorrhiza group.3.Comparison of TTC infarct volume in each group:Compared with model group,infarct size in thrombolytic group and thrombolytic+salvianolic acid group decreased at each time point;Compared with the thrombolytic group,the infarct size of 1d and 3d rats in the thrombolytic+salvianolic acid group was reduced.4.Pathological changes of hippocampal area on the affected side were stained by HE in each group.In the sham operation group,the neuronal cells were regular and the nuclei were organized.In the model group,the morphology of neurons in rat brain tissue was swollen and the nucleus was shrunk.In the thrombolytic group,the arrangement of neuron cells was more regular,but the nucleus shrinkage was more obvious.In salvianolic acid+thrombolytic group,the neuronal cells were arranged more neatly,the neuronal nuclei were basically normal,and the edema of some cells was significantly reduced.Conclusion:1.The changes of eNOS,sTM,sEPCR and other factors in the brain tissue of MCAO autothrombus model rats suggested that cerebral infarction and thrombolysis process of cerebral infarction might lead to vascular endothelial function injury;2.Salvianolic acid for injection can reduce the expression levels of sTM,sEPCR and other factors in promoting blood circulation and removing blood stasis.Early intervention can reduce the expression of vascular endothelial inj ury factor in rats with cerebral infarction.This may be one of the mechanisms of its protective effect on cerebral infarction and ischemia-reperfusion inj ury during thrombolysis.
