Background and objective:Non-alcoholic fatty liver disease(NAFLD)is a liver disease closely related to genetic susceptibility,obesity and metabolic syndrome.It is a pathological state dominated by excessive lipid accumulation in the liver(more than 5% of liver weight),which can evolve from simple fatty liver to cirrhosis and even liver cancer.In recent years,the number of young patients with NAFLD has been increasing year by year.The guidelines for physical therapy or non-drug therapy issued by various countries recommend exercise therapy for NAFLD.Most NAFLD patients have sedentary habits,lack of time to participate in regular physical exercise,less time and can reduce the subjective efforts of the exercise program is desirable.High-intensity interval training(HIIT)has been proved to be more effective than long-term moderate-intensity aerobic exercise in improving blood lipid environment and adipokines such as leptin and adiponectin levels.Whole-body vertical vibration training(WBV),as a time-saving,non-invasive and passive form of exercise,can be used as a safe and feasible alternative to exercise training,and has been shown to reduce visceral fat and liver fat content.Adipocytokines secreted by adipose tissue,such as leptin(LP)and adiponectin(ADPN),can participate in the occurrence and development of IR,while IR can lead to increased levels of free fatty acids(FFA)and converted into triglyceride(TG)deposition in the liver,resulting in lipotoxicity and NAFLD,which plays an important role in the occurrence and development of NAFLD.There are few international studies on WBV combined with HIIT exercise in NAFLD population,and no one has paid attention to the effect of WBV combined with HIIT exercise on adipokines LP and ADPN in patients with nonalcoholic fatty liver disease.Therefore,this study intends to choose WBV combined with HIIT to intervene in obese NAFLD patients,observe the changes of LP,ADPN and liver morphology,and provide a basis for the treatment of NAFLD.Methods:A total of 25 NAFLD subjects who met the experimental conditions were included and randomly divided into experimental group(n = 13,WBV combined with HIIT)and control group(n=12,HIIT).Exercise intervention was 40 minutes each time,3 times a week,for a total of 8 weeks.The effects of two exercise intervention programs on the symptoms and neuromuscular function of NAFLD subjects were observed.Evaluation indicators before and after the experiment: liver fat content(CAP),adipokines:leptin(LP),adiponectin(ADPN),body composition: visceral fat content(VAT),waistto-hip ratio(WHR)and body fat rate(BF%),blood indicators.In this study,SPSS22.0statistical software was used to count and process the data.The data between groups were analyzed by independent sample t test.The data before and after the experiment in the group were analyzed by paired sample t test.Pearson correlation coefficient was used to evaluate the relationship between liver fat content,fat factor,blood lipid content and body composition.P < 0.05 was set as the statistical significance limit.Results:1.Comparison of CAP before and after exercise intervention : Intra-group comparison showed that after 8 weeks of exercise intervention,CAP in the experimental group(p<0.01)and the control group(p<0.05)was significantly lower than that before exercise intervention.There was no significant difference in CAP between the experimental group and the control group(p>0.05).After 4 weeks of follow-up,the CAP content of WBV combined with HIIT group was significantly lower than that of HIIT group(p<0.05).After 8 weeks of exercise intervention,the experimental group recovered 6 people,improved 6 people,the total effective number of 12 people,the overall effective rate(recovery and improvement,the same below)reached 92.3%,significantly higher than the control group without WBV(75%).2.Comparison of Adipocytokines before and after exercise intervention:(1)Leptin(LP): Intra-group comparison showed that after 8 weeks of exercise intervention,LP in the experimental group(p<0.001)and the control group(p<0.05)was significantly lower than that before exercise intervention.Comparison between groups showed that LP in the experimental group was significantly lower than that in the control group(p<0.05).(2)Adiponectin(ADPN): After 8 weeks of exercise intervention,the ADPN of the experimental group(p<0.01)and the control group(p<0.05)was significantly higher than that before exercise intervention;Comparison between groups showed the increase of HDL in the experimental group was significantly higher than that in the control group(p<0.05).3.Comparison of body composition before and after exercise intervention:(1)Body fat percentage(BF%): After 8 weeks of exercise intervention,the BF% of the experimental group(p<0.01)and the control group(p<0.01)was significantly lower than that before exercise intervention.There was no significant difference in BF%between the experimental group and the control group(p>0.05).After 4 weeks of follow-up,there was no significant difference in BF % between the two groups(p >0.05).(2)Waist-to-hip ratio(WHR): After 8 weeks of exercise intervention,the WHR of the experimental group was significantly lower than that before exercise intervention(p<0.05);comparison between groups showed that the experimental group compared with the control group in reducing WHR has significant difference(p<0.05).After 4weeks of follow-up,there was a significant difference between the experimental group and the control group(p<0.05).(3)VAT:After 8 weeks of exercise intervention,the VAT of the experimental group(p<0.01)and the control group(p<0.05)was significantly lower than that before exercise intervention.The comparison between groups showed that the decrease of VAT in the experimental group was significantly different from that in the control group(p<0.05).After 4 weeks of follow-up,the experimental group had significant difference compared with the control group(p<0.05).(4)Waist circumference : After 8 weeks of exercise intervention,the waist circumference of the experimental group(p<0.01)and the control group(p<0.01)was significantly lower than that before exercise intervention.There was no significant difference in waist circumference between the experimental group and the control group(p>0.05).After 4 weeks of exercise intervention,there was no significant difference between the two groups(p>0.05).(5)Hip circumference: After 8 weeks of exercise intervention,the hip circumference of the experimental group(p<0.01)and the control group(p<0.01)was significantly lower than that before exercise intervention.There was no significant difference in hip circumference between the experimental group and the control group(p>0.05).After 4 weeks of exercise intervention,there was no significant difference between the two groups(p>0.05).4.Comparison of blood lipid indexes before and after exercise intervention:(1)Triglyceride(TG): Intra-group comparison showed that after 8 weeks of exercise intervention,The decrease of TG in the experimental group was significantly lower than that in the control group(p<0.05).(2)Low density lipoprotein(LDL): Intra-group comparison showed that after 8weeks of exercise intervention,LDL in the experimental group was significantly lower than that before exercise intervention(p<0.05).Comparison between groups showed that LDL in the experimental group was significantly lower than that in the control group(p<0.05).(3)High-density lipoprotein(HDL): intra-group comparison showed that after 8weeks of exercise intervention,the HDL of the experimental group was significantly higher than that before exercise intervention(p<0.05);there was no significant difference in HDL between the experimental group and the control group(p>0.05).(4)Total cholesterol(TC): Comparison within the group showed that after 8 weeks of exercise intervention,the TC of the experimental group was significantly lower than that before exercise intervention(p<0.05);the TC of the experimental group was significantly lower than that of the control group(p<0.05).Conclusion:The 8-week WBV combined with HIIT significantly improved liver steatosis and blood lipid levels in obese NAFLD college students,with an effective rate of 92.3 %.The effect may be related to reducing leptin production and increasing adiponectin secretion,regulating lipid metabolism,and reducing intra-abdominal fat content.The WBV combined with HIIT training program is feasible,safe,and more effective.It can be used as an optimized exercise method for NAFLD college students to improve lipid metabolism. |
